This letter constitutes our formal reply to the subject Inspection Report. It is our opinion that nothing in your report or in this reply contains any information of a proprietary nature. .

Inspection Report is as follows:

Enforcement Item A, Sub-paragraph A.1 Identification of Design Document Changes as Essential or Non-Essential Check-off blocks to identify a DDC as essential or non-essential were added to the DDC form in February, 1977. The originator is responsible for checking one block. KEI verifies that a block is checked, and does not process the DDC unless this action has been taken. The Cincinnati Gas &

' Electric Company Quality Assurance Engineer verifies the classification prior td approving the DDC. -

Full compliance has been achieved.

Enforcement Item A Sub-paragraph A.3 Physical Requirements and Certification of Inspectors Eye examinations for Kaiser Engineers, Inc. Quality. Control Inspectors have been completed.

Future non-compliances will be avoided by a revision to QAP No. 20 to provide for periodic review of re-examination requirements for all Quality Control Inspectors. This revision will also require a medical examination for inspectors prior to employment, and will delete the requirement for medical examinations on an annual basis.

Kaiser Engineers, Inc. no longer performs NDE on-site, therefore, only their radiographic film interpreters are required to be certified to ASNT-TC-1A.

Full compliance for eye examinations has been achieved. Revision of QAP No. 20 will be completed in June,1977.

Enforcement Item A, Sub-paragraph A.4 Stainless Steel and Low Hydrogen Electrodes Not .

Enforcement Item A, Sub-paragraph A.5 Material Purchased From an Unapproved Supplier An approved revision to QAP No. 6, Supplier Quality Assurance, provides alternate means of approval for suppliers of essential, ncn-ASME Code, material. This revision is consistent with the Kaiser Engineers, Inc. Corporate Quality Assurance Manual and, properly implemented, will preclude future non-c,pmpliances.

I The vendor identified in the inspection report as being unqualified has been surveyed and accepted in accordance with the revision to Kaiser Engineers 3 Inc. QAP #6. This vendor will be added to the Kaiser Engineers,' Inc. approved supplier's list.  ;

The Kaiser Engineers, Inc. Quality Assurance Audit Log Form has been L% vised to provide improved control of any requirement for re-aodit. If required, re-audits will be performed and the re-audit report will be identified with the original audit report number, suffixed with the words

" follow-up". Re-audits will be completed within 30 days.

If the follow-up of an audit deficiency is performed with a Surveillance Report, the report will be identified with the original audit number, suffixed with the words " follow-up" and filed with the original audit. This procedure is in accordance with Kaiser Engineers, Inc. QACMI G-14.

Periodic review of audit logs by the Kaiser Engineers Inc. Quality Assurance Manager will preclude future non-compliances.

Full compliance will be achieved by May 8,1977.
