  • DCR NO. 10CFRSO.59 SAFETY EVALUATION 1ED-0215 This change of impellers will increase the pumps capacity. System will perform original design function and technical specification is not altered, lEC-0555A An unreviewed safety question is not involved as the design change does not alter the existing functional system operation. The change inhances operator ability to facilitate contr.1 options.

1EC-0867 Splitting the lantern ring will not effect the safe operation of the service water pump. The function of the lantern ring is not to provide a water tight seal nor is it a pressure retaining part, therefore, the integrity of the design has not been sacrificed once installed. The lantern ring is restrained in every direction.

Therefore, an unreviewed safety question is not involved.

IEC-0922 This change is not safety related and does not affect safety related equipment. It corrects the indication for an automatic mode. An unreviewed safety question is not involved, lEC-1078 By replacing existing limit switches with Namco EA-180 seriess.dwitches which are qualified for use in the containment, the functional reliability of the switch is improved without compromising its function.

1SC-0074 The providing of coils of a different, more suitable, material does not affect any presently performed safety analysis nor does it create any new safety hazards. The bases of the technical specifications are not affected.

1SC-0302 This change enhances the safety of personnel in case of a CO2 discharge and does not raise any safety question.

1SC-0317 This design change is not safety related and does not in anyway affect any safety related systems or the safe shutdown of the plant.

1SC-0541 This design change revises the loss of AC power alarm for the DC system battery chargers. The charger alarms are not required for the safe shutdown of the reactor.

  • DCR NO. 10CFR50.59 SAFETY EVALUATION ISC-0732 All the existing thimbles are being replaced with new thimbles of a later design, which is considered an improved design. Thimble replacement will also enable complete incore mapping which is presently not possible at the three locations of failed thimbles.

ID of a thimble is less than 3/8" diameter and hence a failed thimble does not constitute a LOCA, by definition. This will minimize challange to ECCS when a thimble fails. A failed thimble can be isolated thru an isolation valve. The same has been proven by plant experience. As such, thimble replacement does not constitue an unreviewed safety question.

IMD-0115 The change involves addition of manual shutoff valves which are normally open. They will be closed only during integrated leak rate test of the containment (ILRT). Administrative controls shall be used to have the valves opened before system operation.

SUMMARY

JULY 1982 7/2 The reactor power was reduced to 57% power at 1700, for the weekend to perform planned plant maintenance. The strainers on the suction pumps, heater drain pumps, condenser pumps and the feedpumps were cleaned. A steam leak on number 11 feedpump was. repaired. Power escalation commenced at 1800 on 7/4 at a rate of 3%/hr.

7/5 The load dispatcher requested power escalation be dis-

  • continued at 0330. The power was held at 82% until 0700 when the load dispatcher requested power escalation.

Power was held at 98% due to low suction pressure on the steam generator feedpumps. Full power was achieved on 7/11.

7/28 At 1225 the load dispatcher requested load reduction to 850 MW in order to maintain voltage stability of the transmission lines because of one line being taken out of service. At 1304 the load dispatcher requested load to be maintained at present level, 911 MW, at 1307 load ,

dispatcher asked for a return to full power. Power was '

increased from 85% at 3% per hour. There was a reactor trip at 1452 from 92% power. An I&C technician pulled the power supply card during a solid state protection surveillance ,

on train "B", This one card is common to both the A and B protection trains, thereby generating a full reactor trip.

The reactor went critical at 2109 and started raising power.
