HIGHLIGHTS OF WEEKLY INFORMATION REPORT Week Ending June 18, 1982 Grand Gulf On June 16 an operating license restricted to 5% of full power was issued for Unit 1.

Grand Gulf is the first BWR with a Mark III containment to be licensed domestically and the third such unit world-wide.

Oconee Unit 3 Oconee 3 shut down April 23 for an extended refueling outage and other work items.

Inspection of the lower thermal shield bolts revealed cracks in 51 of the 96 A286 bolts. Duke Power Company will replace all of the bolts before the unit returns to operation scheduled for September 15, 1982.

Big Rock Point The first phase of the spent fuel pool expansion hearing for the plant concluded on June 12. About half a dozen issues, including adequacy of emergency plans, have been deferred until the next hearing session which has not yet been scheduled.

South Texas Project Non-safety related construction resumed on June 15. By letter (5/14),

Houston Lighting and Power Company requested approval from Region IV to resume safety-related construction on July 15, 1982.

CRBR Fuel Cycle Environmental Review NHSS received and is reviewing DOE's Amendment XIV to the CRBR Environmental Report. All fuel cycle and safeguards sections for the updated FES were completed and delivered to the CRBR Project Office on June 10, 1982.

LOFT _

The Large Break Test (L2-5) was conducted in the LOFT reactor in Idaho on June 16.

Initial observed data seems to coincide with the test predictions and early indications.are that the test was successful.

Nuclear Property Insurance On June 15, the EDO approved. an advance notice of proposed rulemaking to amend 10 CFR Part 50. This notice requests comments on a rbport on nuclear property insurance prepared by Dr. John Long of Indiana University.

Hydrogen Burn The first series of Hydrogen Burn Survival (HBS) Program tests are now under-way at Sandia's Fully Instrumental Test System (FITS).

Facility security approval was also granted to Rockwell International for their Canoga Park, CA, fac ility.

Security Support Services for ASLBP Hearings SEC provided security advice and assistance with respect to ASLBP Shoreham and Seabrook hearings.

OFFICE OF NUCLEAR REACTOR REGULATION ITEMS OF INTEREST Week Ending June 18, 1982 Oconee Nuclear Station, Unit 3 Oconee 3 shutdown on April 23, 1982 for an extended refueling outage.

Work items to be accomplished include repali of the SGs' internal AFW header, Appendix R and 0737 modifications, SG cavity tendon replacement and the ten year ISI.

Following RPV internals removal, inspection.of the lower thermal shield bolts revealed cracks in 51 of the 96 A286 bolts (UT indications of 15 to50%).

Duke Power Company will replace all.of.the lower thermal shield bolts with a Inconel X-750 studs and nut assembly, utilizing the base plate locking device and crimp cup,.similar to.the modifica-tion made on Units 1 and 2 to correct comparable degradation.

Unit 3 is scheduled to return to operation on September 15, 1982.

Grand Gulf On June 16, 1982 an operating license restricted to 5% of full power was issued to Mi'ssissippi Power and Light Company, Middle South Energy, Incorporated, and South Mississippi Electric. Power Association for the Grand Gulf Nuclear Station, Unit 1.

Grand Gulf is the first BWR with a Mark III containment to be licensed domestically and the third such unit world-wide.

Big Rock Point The first phase of the. spent. fuel pool expansion hearing for Big Rock Point concluded on Saturday afternoon, June 12, 1982.

The following issues have been deferred by the Board until tiie next hearing session which has not yet been scheduled:

1.

Structural integrity of spent fuel pool at boiling temperatures and with point loading from the storage rack feet.

2.

Seismic qualification of the semi-gantry crane i ;ide the con-tainment.

3.

Risk to the spent fuel pool of military and general aviation overflights and crashes.

4.

Adequacy of containment shielding under fuel pool accident con-o ditions.

5.

Adequacy of the Big Rock Point emergency plan.

Possib*ility of a zircaloy-water reaction which might become self-sustaining if the pool begins boiling.

South Texas Project Houston Lighting and Power Company (HL&P) informed NRR that non-safety related construction resumed on June 15.. By letter of.May 14, 1982, HL&P requested approval from Region IV to resume safety-related construction on or about July 15, 1982.

R. E. Ginna Nuclear Power Plant The R. E. Ginna Nuclear Power Plant will reduce power.to 50% tonight.in order to remove a loose part from the inlet side of Main Condenser Water Box 2B2.

The loose part was detected by the STA while touring the plant.

The loose part can be heard when standing next to the water box inlet and from the sounds it is making it appears to be small.

After reducing power to 50%, the.2B2 water box (1 of.4 water boxes) will be opened, inspected for damage and the loose part retrieved. The plant expects to return to full power by tomorrow (June 17,1982) afternoon.

The resident inspector is following the situation closely.

Effluent and Environmental (Radiological) Information Liquid effluents from the TMI site released to the Susquehanna River after processing, were made within the regulatory limits and in accordance with NRC requirements and City of Lancaster Agreement dated February 27, 1980.

During th'e period June 11, 1982, through June 17, 1982, the effluents contained no detectable radioactivity at the discharge point although individual effluent sources which originated within Unit 2 contained small amounts of radioactivity. Calculations indicate that less than five-thousandths (0.005) of a cur.ie of tritium and less than two hundred-thousandths (0.00002) of a curie of cesium were discharged.

Airborne Effluents Airborne releases to the environment as measured by licensee installed monitors at discharge stacks are listed below. These releases were well within regulatory limits.

Environmental Protection Agency (EPA) Env'ironmental Data The EPA Middletown Office has not received the environmental Kr-85 dnalytical results for the samples which were taken May 21, 1982 through June 4,1982, from the EPA's Counting Laboratory at Las Vegas, Nevada.

These results will be included in a subsequent report.

No radiation above normally occurring background levels was detected in any of the samples collected from the EPA's air and gamma rate networks during the period from June 9, 1982.through June 17, 1982.

TMI Occupational Exposure Licensee TLD (Thermoluminescent Dosimeter) records indicate the following Unit 2 occupational radiation exposure for 1982:

Thus, if each member of a population group of 1,000 people were to receive a dose of 0.001 rem (1 millirem), of if two people were to receive a' dose of.0.5 rem (500 millirem) each, the total man-rem dose in each case would be one man-rem.

Licensee Radioactive Material and Radwaste Shipment On Thursday, June 17, 1982, 50 drums containing Unit 1 and Unit 2 contaminated laundry were shipped to Tri-State Industrial Laundries, Utica, New York.

Major Activities Submerged Demineralizer System (SDS).

Processing of the second batch 1.

(approximately 50,000 gallons) of reactor coolant system (RCS) water was completed on June 12, 1982.

Pumping of reactor building sump water to the SDS feed tanks commenced on June 14,1982, and was terminated on June 15, 1982, when an SDS sand filter became loaded. Approximately 4,000 gallons were transferred.

The water is being pumped from a point below the general floor level (in the incore instrument trough) of the reactor building and apparently sludge is being washed into this area, drawn into the pump suction, and collected on the sand filter.

Including this 4,000 gallons, the total amount of sump water in the SDS feed tanks is now approximately 47,000 gallons. This water remains to be processed.

EPICOR II. The EPICOR II system is shutdown on standby status.

3.

Reactor Coolant System (RCS) Feed and Bleed.

The third feed and bleed cycle of the RCS commenced on June 14, 1982 and was completed on June 17, 1982.

4.

Reactor Building Entry.

During the reactor building entry on June 17, 1982, a radiation survey was made in the open stairway going down to the 282 ft. elevation. A health physics technician descended the stairs from the 305 ft. elevation to a landing approximately eight feet above the basement floor (282 ft. elevation).

Radiation levels at the landing were 12 R/hr gamma and 35 rad /hr beta. A radiation survey approximately three feet above the basement floor indicated that the gamma field was 24 R/hr.

Based on a visual observation, the basement ENCLOSURE B

floor was described as completely covered with a liquidity brown material. The depth of the material was difficult to estimate, and may range from a fraction of an inch to several inches.

Two reactor building entries are scheduled for next week.

During the above steps, collection of current, voltage, and accoustic data will be made to help determine the APSR mechanism performance characteristics.

Following these steps, the APSR will be driven into its " full-in" position such that normal uncoupling of the APSR from its lead screw can be performed prior to reac, tor vessel head removal.

If at any time the APSR fails to move at the maximum electrical current that can be applied to the drive mechanism, the APSR will be left in that position and attempts will be made to move the next APSR until all eight have been dther moved to the " full-in" position or it has been verified that the The data gathered during this evolution is expected APSR will not move.

Week Ending June 18, 1982 CRBR Fuel Cycle Environmental Review DOE's Amendment XIV to their CRBR Environmental Report was received June 9.

It is now under review by the NMSS staff and BPNL.

All fuel cycle and safeguards sections for the updated CRBR FES were completed and delivered to the CRBR Project Office on June 10, 1982.

The last round of Discovery items (Set #25) have been received.

It is expected that all staff responses will be completed in time to provide inputs to ELD by June 14.

New Guidance Document Issued A guidance document, designated NUREG-0923, to aid licensees in carrying out NRC rule for advance notification of governors (or state officials) concerning imminent shipments of spent fuel and radioactive waste has been issued and will be distributed commencing June 18, 1982.

Consulting to the IAEA During June 24-July 7, an SGMT staff member will be consulting with the Safe-guards Evaluation Section of the IAEA to develop a Quality Assurance Program for containment and surveillance devices used by safeguards inspectors in the l

field and at Headquarters.

EN-82-19, Southern California Edison Company (San Onofre) - A Notice of Violation and a Notice of Proposed Imposition of Civil Penalty in the amount of $60,000 were issued on June 17, 1982. The action was based on violations relating to positive access control to vital areas associated with the implementation of the physical security plan.

EN-83-29A, John C. Haynes Company (Heath, OH) - An Order Rescinding the Order to Modify License (initial Order dated August 28, 1981) was issued on June 15, 1982.

The recission of the Order to Modify License is based on the satisfactory corrective action taken by the licensee.

0FFICE OF NUCLEAR REGULATORY RESEARCH Items of Interest Week Ending June 18, 1982 Hydrogen Burn The first series of Hydrogen Burn Survival (HBS) Program tests are now underway at Sandia's Fully Instrumental Test System (FITS). The series started with 6 percent hydrogen burns and will continue through progressively more severe burns up to 15 percent. Tests with 0, 20, and 40 percent steam are included. The purposes of the tests are:

(1) to develop instru-mentation, and (2) to characterize the hydrogen burn environment'for the purposes of developing and verifying analytical models.

On June 16, 1982, a briefing was given to Jim Milhoan of OPE, Roger Strehlow OPE's Consultant on Hydrogen and several members of the Containment System Reseach/NRR on the RES Hydrogen Safety Research Program by John Larkins.

The briefing was requested by OPE to assist them in assessing the status of work (both RES and NRR) on hydrogen particularly as it relates to ice condenser containments.

LOFT Large Break Test Completed The L2-5 experiment was conducted in the LOFT reactor in Idaho at 6 p.m. local time on Wednesday, June 16. This experiment simulated a 200% (gillotige) break in a reactor cold leg pipe. The peak clad temperature was about 1480 F.

An effort was made to simulate rapid pump shutdown by shutting off the current to the M-G set in i second such that the coast down was reduced to less than a minute from the normal coast down lasting several minutes. This resulted in no early quench such as was seen in previous large LOCA experiments. Quench was complete in 53 seconds (initial quenching was seen in less than ten seconds in previous experiments).

Low pressure injection was stopped at 100 seconds in an unexnected procedure initiated by EG&G in preparation for the final LOFT test L2-6 in which fuel damage is expected. This procedure was done to ten if refill water could be confined to the vessel without spilling out into the rest of the system, to be able to confine the fission products to the ve.ssel in L2-6.

As a result of this procedurg a second core dryout was experienced in which clad temperatures of about 1100 F were seen.

The initial observed data from L2-5 seems to coincide with the test predittions and early indications are that the test was successful.

. Meeting with Dr. John R. Lakey Dr. John R. Lakey, Nuclear Department Director at the Royal Naval College, United Kingdom, met with various personnel in RES June 21 and 22,1982.

The subjects covered included radiation health effects, occupational radiation protection, emergency planning, and decommissioning.

Request to Utilities to Participate in Research Studies on Pressurized Thermal Shock (TAP A-49) and Safety Implications of Control Systens JIAPA-47)

On June 15 and 16 representatives of RES and NRR visited Baltimore Gas and Electric and North Carolina Power and Light.

We asked them to provide data from Calvert Cliffs and H. B. Robinson for the ORNL probabilistic study of pressurized thermal shock and the related ORNL study of safety implications of control systems.

Both utilities agreed to consider our request and reply in about a week.

ITEMS OF INTEREST 0FFICE OF INTERNATIONAL PROGRAMS WEEK ENDING JUNE 18, 1982 NRC-Dutch Cooperation IP was advised this week that the Netherlands Ministry for Social Affairs and Employment, which regulates the country's nuclear power industry, has accepted (with two minor modifications) the information exchange and cooperation renewal Arrangement proposed by IP letter dated April 29, 1982.

The present Arrangement would have expired October 3,1982. The NMSAE hopes to have a Commissioner visit The Hague for a signing ceremony.

IP is working on this now.

Foreign Visits to NRC On Monday Dr. Dan Litai, Director of the Licensing Division of the Israeli Atomic Energy Commission (IAEC) and the Israeli Administrator of the NRC-IAEC information exchange and cooperation Arrangement, met with represent-atives from OPE and NRR to discuss Israeli-developed safety goals, demo-graphic siting criteria, and the NRC QA-NDT requirements during design procurement, construction and operation stages.

On Wednesday Mr. Tom Moss of the Australian Atomic Energy Commission's Regulatory Bureau met with Frank Coffman and Sandy Israel, NRR and Gary Burdick, RES.to discuss the use of fault and event tree modelling techniques for PRA.

On Wednesday Mr. Jacques Furet, Head of the Department of Nuclear Safety of the French Commissariat a l'Energie Atomique (CEA) discussed control rooms, emergency operating procedures, criteria and development of emergency response with D. Zieman, M. Goodman, V. Moore, L. Beltrachi, J. Buzy, NRR and B. Grimes, IE.

On Friday a 5-member Korean delegation, headed by Dr. Kong, Bak Kwang, visited NRC to discuss international cooperation in nuclear safety train-ing and cooperation in the event of a nuclear emergency. On Thursday the delegation met with representatives from the Departments of State and Energy to discuss a proposed joint study on spent fuel management. This visit was a preparatory meeting of the Joint Standing Committee on Nuclear and Other Energy Technology (JSCN0ET) prior to the formal JSCNOET meeting to be held in Washington this fall.

Washington State Liaison Officert Meeting Nicholas Lewis, the Governor-appointed State Liaison Officer for Washington, accompanied.by Frank Young of OSP, met with members of the Licensing Board Panel on June 16th to discuss minor amendments to the Washington State Agreement with NRC.

On June 17th Mr. Lewis met with William Dircks, ED0; John Davis and John Martin, NMSS and Jerome Saltzman, OSP to discuss the high level waste issues in the State of Washington.

Interstate' Low-Level Radioactive Waste Compacts A number of States are negotiating in more than one compact group resulting in the compacts' membership being fluid at this time (see

The chart on the following page reflects not only current eligibility in each compact, but also legislative status, and will be published periodically to reflect any changes in information. Substantial State legislative activity in 1983 is expected. All compacts require Congressional ratification to be fully effective.

~New Mexico Review The State of New Mexico's radiation control program will be reviewed beginning June 21, 1982.

Resolution - Foreign Uranium Imports'and Mill Tailings OSP has learned that the Western Governors' Conference has passed a resolution urging adoption of Senator Domenici's proposed amendments to NRC's Authorization Bill to. impose a 20% restriction on foreign uranium imports and to diarify Ag'eement State authority over mill tailings.

Both units continued to operate throughout the week at or near full power

IBEW members started returning to work on June 11.

Returning workers have been phased back into regular duties and normal operations have resumed. The licensee's post strike security program remains in effect.

Licensee post-strike assessments of worker attitudes by management are nearly completed. Twenty-four hour NRC inspection coverage by both reactor operations and physical protection inspectors has continued.

O REGION V ITEMS OF INTEREST WEEK ENDING JUNE 18, 1982 Environmental Protection Agency Corvallis Environmental Research Laboratory On 6/7/82, a new detector cell containing approximately 15 millicuries of nickel-63 was installed in gas chromatograph. On 6/8/82, a temperature of 470 degrees C was read on the gas chromatograph which was observed to exceed the normal operating temperature of the detector cell of less than 375 degrees C.

The licensee's Radiation Safety Officer was notified and the laboratory containing the gas chromatograph was sealed off from use until the room was monitored for contamination. On 6/9/82, wipes were taken throughout the laboratory and assayed by proportional and liquid scintilla-tion counting techniques. The floor and bench top surfaces were found to be contaminated with an unidentified isotope with an energy characteristic of hydrogen-3.

The licensee reported that no other isotopes have been or are currently used in the laboratory except for nickel-63 used in the detector cell. A maximum of 12,000 cpm was measured from the contaminated wipes. The laboratory and gas chromatograph remain unused until decontamination pro-cedures currently in effect reduce the contamination present to acceptable levels. A urine bioassay has been performed on one individual. Results of the bioassay are unavailable to date.

No bioassay has been performed on a second potentially exposed occupational worker.

Southern California Edison Company San Onofre DN 50-206 On June 17, 1982, the Region V Administrator notified the licensee of the proposed imposition of a $60,000 civil penalty for Severity Level III violations of physical security requirements.

San Onofre DN 50-361 The resident inspector reported that, during the conduct of a special l

Auxiliary Feedwater System (AFW) test on June 1, 1982, to determine the cause of previously observed piping vibrations, a weld attaching a one inch vent line to a two inch line failed. The failed weld is on a crossover line connecting the discharge lines of the motor driven AFW pumps. The failure resulted in an accumulation of non-radioactive water in the AFW pump rooms.

The licensee repaired the failed weld, performed dye-penetrant testing of 13 other welds, and is continuing with post-core hot functional testing. After investigation of the event, the licensee attributes the excessive piping vibration to use of a flow patch which resulted in flow in a reverse direction through a Kerotest valve.

' Portland General Electric Company Trojan - DN 50-344 After removal of the core barrel from the reactor vessel, remote video examination of metal pieces found in the bottom of the reactor vessel, together with UT measurement results, indicate that the pieces are probably l

portions of thermal sleeves initially installed where the safety injection accumulator piping joins the reactor coolant system cold leg piping.

Pieces representing about 1 and 1/4 sleeves have been found.

UT results obtained from the nozzles and compared with UT results from a mockup suggest that all four sleeves are missing. However, because of the inherent difficulty in interpreting UT results, the licensee is also planning video inspection of l

the cold legs and nozzles. Based on discussions with Westinghouse, the licensee concludes that the thermal sleeves are not really required (not installed in newer plants) and does not plan to replace them. Specific corrective actions have not been determined by the licensee.
