Based upon the results of the NRC inspection conducted on April 20-21, 4

1978, certain activities under your license appear to be in noncompliance with NRC requirements as identified below:

1.

Contrary to License Condition 11, individuals who used two 40 gram Pu 238 encapsulated heat sources beginning in March,1978, were not designated by your Ionizing Radiation Control Committee and the Radiological Safety Officer.

Contrary to License Condition 12C, leak tests conducted on or j

i about March 22,1978 of two 40 gram Pu 238 encapsulated heat sources revealed the presence of removable alpha activity 3

exceeding 0.005 microcuries and the sources were not with-drawn from use, and no report of the leaking sources was filed with the NRC.
