The following information is submitted concerning corrective measures

taken in response to the Nuclear Regulatory Commission license infractions j
noted in reference (a):
a.
Item 1.
The two 40 gram Pu-238 heat sources cited in reference (a) have been removed from service and are being returnec' to the sponsoring.

The actions and procedures taken to remove the sources received l
prior approval of the Ionizing Radiation Control Committee and the Radio-r logical Safety Officer.
Future operations involving licensed material r
will receive similar review and approval.
It is considered that full 4
3 compliance has been achieved as of the date of this letter.

Item 2.
License Condition 12C requires the application of leak

test standards to encapsulated sources.
While cladding on the sources was intended to reduce external contamination levels, it was not meant k
to provide full encapsulation. The license application did not specify y
source encapsulation nor did the license issued by.the Nuclear Regula-tory Commission identify the sources as encapsulated.
Upon receipt of Q
reference (a) which described the sources as encapsulated, they were removed from service (10 May 1978).

Item 3.
The subject air samples taken on 23 March 1978 were counted on 24 March 1978 and determined to be below the limits specified in 10 CFR 20,103.
The results were not logged immediately and sub-sequently became forgotten until April 1978.
In the future all counting and evaluation results will be logged immediately upon receipt.
2.
Steps to improve management controls over the radiation safety pro-gram will include more active participation by the Ionizing Radiation Control Committee and additional trainin iological safety by Safety Group personnel.
