SUMhRY OF THE OPERATING REACTOR EVENTS MEETING ON JUNE 24, 1981 Gn June 24,1981, an Operating Reactor Event meeting was held to brief the Office Director, the Division Directors, and the'ir represen-tatives on events which occurred since the last meeting. The list of neeting attendees is included as Attachment 1.

a The events discussed and the significant elements of these events are presented in Attachment 2.

In addition to these events, the assignment of follow-up review responsibility was discussed.

The assignments made during this meeting and the status of previous assignments are presented in Attachment 3.

The next Ope, rating Reactor Events meeting is scheduled for July 1,1981.

The following Notification of Significant Enforcement Action was dispatched during the past week:

a.

EN-81-21 flagnaflux Corporation, Chicago, IL - A Notice of Violation and Proposed Imposition of Civil Penalties in the amount of $8,000 was issued on June 17, 1981, to subject licensee. This action was based on alleged violations involving a radiation dose to a radiographer in excess of NRC requirements, failure to conduct radiation surveys of a radiography exposure device after each radiographic operation, failure to have installed a radiation monitoring system interlocked with the entrance to a permanent radiographic facility to warn of high radiation, and failure to calibrate a radiation survey instrument at the required intervals.

IE Information Notice 81-18, " Excessive Radiation Exposures to the Fingers of Three Individuals Incurred During Cleaning and Wipe Testing of Radioactive Sealed Sources at a Sealed-Source Manufacturing Facility", was transmitted on June 12, 1981, for issuance on June 23, 1981 to selected licensees whose programs may involve potential for high exposures of the fingers or hands of employees.
