This refers to the safety inspection conducted by Messrs. J. Thomas, S. Reynolds,
and W. Rekito of this office on August 27 - October 19,1981 of activities relating
to the failures of two Shutdown Cooling System heat exchangers and to the discus-
sions of our findings held by Mr. Thomas and other members of this office with Mr.
J. Carroll and other members of your staff periodically during and at the conclu-
sion of the inspection. This also refers to the meeting held at the Oyster Creek
Nuclear Generating Station on September 10, 1981. This meeting was attended by
you, Mr. Carroll, and other members of your staff and by Mr. Allan and other mem-
bers of this office.
Areas examined during this inspection are described in the NRC Region I Inspection
Report which is enclosed with this letter. Within these areas, the inspection con-
sisted of selective examinations of procedures and representative records, inter-
views with personnel, and observations by the inspector.
An apparent Violation of NRC requirements is cited in Appendix A and assigned Se-
verity Level IV in accordance with Federal Register Notice 45 FR 66754 (October 7,
1980). A reply is requirM and should be prepared in accordance with Appendix A.
It is not subject to the clearance procedures of the Office of Management and Bud-
get under the Paperwork Reduction Act of 1980, PL 96-511.
This transmittal will be made public unless you contact this office within 10 days
of the date of this letter and submit written application to withhold infomation
within 30 days of the date of this letter. Such application must comply with 10 CFR2.790(b)(1).
