The U.S. Nuclear Regulatory Commission recently receised information concerning acusiti at the Millstone Nuclear Power Facility. Unit 2. The details are enclosed for your resiew an followup.

We request that the results of your review and disposition of these matters be submitted to Region I within 30 days of the date of receipt of this letter. We request that your response contain no personal privacy, proprietary, or safeguards information so it can be released to the public and placed in the NRC Public Document Room. If necessary, such informat shall be contained in a separate attachment which will be withheld from public disclosure.

The affidavit required by 10 CFR 2.790(b) must accompany your response if proprietary information is included. Please refer to 6!e number RI-91-A-014S when providing your response.

The enclosure to this letter should be controlled and distribution limited to personnel with a "need to know" until your insestigation of the concern has been completed and reviewed by NRC Region I. The enclosure to this letter is considered Exempt from Public Disclosure Howes er. a copy of i

Issue On June 14.1991, differences between a control wiring diagram and actual plant conditions was found. The wiring diagram (Drawine 25203-32020) for the No. I A-A' Steam Generator hiain Steam Bypass Valve (2-h1Sd5A) hiotor Operator indicates the p

presence of an amber light to signify when the unit has tripped on overload. This 4

light is not installed.

Also, Contact 49, shown on Drawing 25203-32031, sht. 42 does not exist.

Request Please discuss the validity of the above assertions. If deficiencies are found in plant drawings, please notify us of the corrective actions taken to prevent recurrence.

Please provide us with an assessment of the significance with regard to safety of any identified deficiencies.
