_Sb1 MARY OF PLANT OPERATIONS On Nov. 20, 1982, the plant went to a cold shutdown condition for OTSG tube plugging repair. This repair was completed on Dec.11 (see Attachment A - 0TSG Tube Inservice Inspection).
On Dec.13 the Reactor was brought critical and the generator went on line.
With the exception of several delays due to minor maintenance problems, reactor power continued to be increased until Dec. 20; at this time the reactor was at 90% full power.
Reactor power was maintained at this level (with the 1B/2B Feedwater Heaters out of service).
On Dec. 22 the reactor was tripped as a result of a major grid disturbance caused by offsite downed power lines.
On Dec. 26 the reactor was brought critical and the generator on line.
Reactor power continued to be increased until Dec. 27; at this time the reactor was at 60% full power.
For the remainder of the month the reactor power sas maintained at this level at the request of the load dispatcher.
_P_ERSONNEL CHANGES REQUIRING REPORT:
In accordance with Technical Specifications Figure 6.2-2, the following two personnel changes are hereby reported.
1.
Mike Oren has been promoted from Associate Nuclear Engineer to Senior Nuclear Engineer. Mr. Oren's experience and education are as follows:
Experience:
SMUD, Rancho Seco: From 11/78 to Present.
Westinghouse-Hanford: From 7/75 to 11/78; responsibilities with LMFBR Safety Development Program; Safety test design and analysis of results as verification and development of LMFBR Safety Analysis Codes.
Babcock & Wilcox:
From 4/73 to 7/75; responsibilities with the ECCS performance analysis group; performed ECCS analyses for inclusion in the utility FSARs and topical reports.
Education:
M.S.N.E., University of Missouri, Columbia, 4/73 B.A. (Physics Major), University of Northern Iowa, 6/70 2.
B. Wichert has been promoted from Associate Mechanical Engineer to Senior Mechanical Engineer. Mr. Wichert's experience and education are as follows:

From 12/76 to Present. Served as Engineering Student /
Clerk for one year. Assistant Mechanical Engineer for two years and six months.

SUMMARY
OF CHANGES IN ACCORDANCE WITH 10'CFR 50.59 (b)
Three (3) changes were completed.in December 1982.
1)
The Spray Pond Buildingswere constructed to enclose the pumps and valves adjacent to the ponds. The Valve Building is a reinforced concrete structure. The Spray Pond Pump Building is steel, louvered for natural ventilation of the pumps.
The Buildingswere designed to seismic Category I structural criteria and for natural draft cooling to protect the equipment as required.
2)
The "A" 0TSG Internal Auxiliary Feedwater Header was found to have a crack, approximately 20 inches long, in the lower inside weld.
The stabilized Internal Auxiliary Feedwater Header will withstand the highest postulated loads which result from a Main Steam Line break.
A Finite Element Analysis Model has taken the 20-inch crack into account and has snown the header to be acceptable. However, the header, while still in place, has been removed from service.

The "B" 0TSG Internal Auxiliary Feedwater Header was found to have a crack, approximately 15 inches long, in the lower inside weld.
The stabilized Internal Auxiliary Feedwater Header will withstand the highest postulated loads which result from a Main Steam Line break. A Finite Element Analysis Model has taken the 15-inch crack into account and has shown the Header to be acceptable. However, the header, while still in place, has been removed from service.
MAJOR ITEMS OF SAFETY RELATED MAINTENANCE 1.
Replaced stator on control rod #3, group 6.
2.
Replaced auxiliary contact on Decay Heat Cooler Outlet valve.
3.
Welded leaking pipe between DHS-523 and 26021-10"-GD, Decay Heat Removal System.
4.
Constructed and installed a remote computer read-out for Bailey 855 operator's console.
5.
Cleaned leaking valve - Nitrogen to Makeup Tank Check Valve.

Steam Generator Tube Inspection Results On Saturday, November 20, 1982, at approximately 11 p.m.,
Operations' personnel at Rancho Seco became aware of a Primary to Secondary leak in the "A" Steam Generator.
The plant was shut down in order to repair the leak.
The leaking tube was identified as Tube Number 77-2.
This tube is adjacent to the untubed inspec-tion lane in the Steam Generator.
Eddy Current testing of approximately 1280 additional Steam Generator tubes identified one additional tube which showed a 46% through-wall indication.
Both this tube, Number 74-17, and the leaking tube have been stabilized and plugged.
NOTE:
In Accordance with Technical Specifications 6.9.4.1.c, the occurrence described above has also been reported as Licensee Event Report Number 82-31.
