Brand-Rex Company, Willimantic, CT 06226 INVESTIGATION a)
Brand-Rex Company manufactured and shipped five (5) reels of 3 conductor #12 AWG to Sacramento Municipal Utility District, Rancho SECO Nuclear Station #1 during 1979.
b) One 8 inch manufacturing splice was detected within cable at time of installation.
c) Splices are used in the manufacturing operation to facilitate lengths.
d) Brand-Rex Company developed and implemented a splice procedure prior to 1979 and formalized it during 1981.
e) Copies of approved Brand-Rex manufacturing splices procedures are in attachment.
CAUSE OF FIELD PROBLEM a) Manufacturing operator did not follow procedures applicable to the construction of proper splices.

,. CORRECTIVE ACTION a) Manufacturing has conducted training classes applicable to splicing techniques in areas where splicing is used.
b) Training program referenced above has been documented.
-c) Cable contained within Hold Status will be returned to Ber.nd-Rex for examination to ensure the absence of splices.
