81-050/03L-0 D'E!.T DATE:

08-04-81 TITLE OF THE EQT:

2-CH-P-1 A INOPERABLE 1.

EVENT DESCRIPTION

With the unit at full power, "C" charging pump out of service for minor maintenance, and "A" charging pump in operation, it was discovered during a routine walkdown that oil was being slung from the outboari becting of the "A" pump. The "B" charging pump was started and "A" was removed from service leaving only one pump operational. This eve.nt is contrary to T.S.-3.2.C.1 and is reportable per T.S.-6.6.2.b(2).

2.

, PROBABLE CONSEQUENCES:

The charging pumps supply makeup water to the reactor coolant system, RCP seal injection, as well as high head safety injection. One pump,_however, is sufficient to supply 1007. of the charging and high head safety injection require-ments.

In accordance with T.S.-3.3.B.2, two pumps were operable within 24 hours2.777778e-4 days
0.00667 hours
3.968254e-5 weeks
9.132e-6 months
; therefore, the health and safety of the public vere not affected.

3.

CAUSE

The "A" charging pump was declared inoperable when it was found to be slinging oil from its outboard bearing. This was a result of the failure of the bearing seal as result of water in the lube oil. The water entrained in the lube oil originated from a tube failure in the charging pump lube oil cooler.

4.

IFDfEDIATE CORRECTIVE ACTION:

The immediate corrective action was to start the "B" charging pump and immediately begin corrective maintenance on the "C" charging pump.

5.

SUBSEQUENT CORRECTIVE ACTION:

The "C" charging pump, which had a minor seal leak, was returned to operable status within 24 hours2.777778e-4 days
0.00667 hours
3.968254e-5 weeks
9.132e-6 months
. The "A" charging pump bearing seal & lube oil coolsr were substquently repaired 6 the pump returned to service.

6.

ACTION TAKEN TO PREVEhT R_ECURRENCE:

None deemed necessary.

7.

GENERIC IMPLICATIONS:

None.
