Pressurizer Overpressure Protection System Channel 2 - Inoperable.

This report was initiated by Incident Report 81-242.

CONDITIONS PRIOR TO OCCURRENCE:

Mode 4 - Rx Power 0% - Unit Load 0 MWe DESCRIPTION OF OCCURRENCE:

During cooldown on July 9, 1981, the Pressurizer Overpressure Protec-tion System (POPS). channel 2 was declared inoperable due to previous closure of isolation valve 2PR7 because of known leak through on the pressurizer power operated relief valve (PORV) 2PR2.

Action statement 3.4.10.3.a was entered when Reactor Coolant System (RCS) temperature decreased to 312*F at 1957 hours0.0227 days <br />0.544 hours <br />0.00324 weeks <br />7.446385e-4 months <br />, July 9, 1981.

This occurrence constituted operation in a degraded mode in accordance with technical specification 6.9.1.9.b.

DESIGNATION OF APPARENT CAUSE OF OCCURRENCE:

1 PORV 2PR2 was previously identified to be leaking through, and was isolated by closing 2PR7.

When RCS temperature was decreased to 312*F, POPS channel 2 was declared inoperable because it was isolated.

ANALYSIS OF OCCURRENCE:

Technical Specification 3.4.10.3.a requires:

With one POPS inoperable, restore the inoperable POPS to operable status within 7 days or depressurize and vent the RCS through a 3.14 square inch. vent within the next 8 hours9.259259e-5 days <br />0.00222 hours <br />1.322751e-5 weeks <br />3.044e-6 months <br />.

CORRECTIVE ACTION

At 0715, July 12, 1981, Unit 2 was raised above 312*F, terminating action statement 3.4.10.3.a.

Action statement 3.4.5.a was entered immediately because 2PR7 was closed and the power removed.

Valve 2PR2 will be repaired and tested during the next refueling outage.

A supplemental report will be submitted at that time.
