CAUSE DESCRIPTION AND CORRECTIVE ACTIONS (Continued) the strainer; therefore, the plant operator placed the mode selector switch in Mode 3.

The Shift Supervisor had intended to simply stop the D.G. and i

clean the strainer in a routine manner. During the shift turnover, the on-coming Shif t Supervisor obaerved the position of the : node selector switch and it was placed in Mode 1 and D.G. 1-2A was returned to service at 2300 on 5-29-81.

Individuals involved have been counseled regarding this event in order to prevent recurrence.
