June-16, 1981 Identification of Occurrence:

The cooling fan, AHF-15B, for Decay Heat Closed Cycle Cooling Pump 1B was declared inoperable. This event was contrary to Technical Specification 3.7.3.2 and Technical Specifica--

tion 3.5.2.

Conditions Prior to Occurrence:

Mode 1 power operation (100%)

Dascription of Occurrence:

At 0530 during normal operation it was discovered that AliF-15B, the cooling fan for. Decay Heat Closed Cycle Cooling Pump 1B was excessively noisy. The fan was shut down and maintenance was initiated. Operability was restored at-1021 on June 17, 1981.

Designation of Apparent Cause:

The cause of this event is attributed to worn bearings in the fan motor.

Analysis of Occurrence:

Redundancy was provided by the "A" ECCS Subsystem. There was no effect upon the health or safety of the general public.

Corrective Action

The sealed bearings were replaced. The sealed bearings.were also replaced in AHF-15A as a preventive measure. No further corrective action is deemed necessary at this time.

Failure Data:

This was the first occurrence for AHF-15B and this is the first event reported under Technical Specification 3.7.3.2; the fifteenth under Technical Specification 3.5.2.
