EVENT DESCRIPTION

On June 10, 1981, the "B" Diesel Fi re Pump failed to pass the Monthly Fi re Pump Capacity Test, 405 4100-7 The fi re pump was taken out of service on June 11, inspected, and re-tested with the pump capaci ty remaining unchanged.

The pu,; was then removed from service for maintenance.

On June 18, 1981, with both Units running, the limitation of 7 days was reached, for having the "B" Diesel Fi re Pump out of service. Technical Speci fication 3.12.B.2 requi res a report to be submitted to the commission, explaining the back-up protection which replaces the systems' loss of redundancy.

VI.

PROBABLE CONSEQUENCES OF THE OCCURRENCE:

While the "B" Fire Pump is inoperative, fire protection water will be provided by the "A" Diesel Fire Pump and a 3160 GPM portable Diesel Fire Pump.

The portable fire pump is located at the discharge flume with all hardware to readily tie into the Fire Suppression System. Another portable fire pump is on site and can be placed in parallel with the first portable fire pump to provide additional pumping capacity should the need arise.

The portable fire pump was tested and proven operable prior to June 18, 1981. A temporary procedure was written for operation of the portable fire pumps. This system will be maintained until the "B" Fi re Pump is returned to service and tested.

CAUSE

Upon disassembly of the "B" Diesel Fi re Pump, wood debris was found in the suction piping. The water source for the pump is the Mississippi R i ve r,

which contains some debris, which can be pulled into an operating pump.

These pumps normally have a bronze strainer which covers the suction.

When the pump was removed for service, no such strainer was found. This is the second occurrence of wood blocking the suction of a pump.,

On April 4,1981, a RHR Service Water Pump failed a flow test due to wood in the suction piping.

CORRECTIVE ACTION

The pump was removed to the Maintenance Shop for repairs.

The pump was dis-assembled, the wood removed and the pump inspected for damage.

No damage to the pump was found. While the pump is disassembled, new rubber seals will be installed. To prevent similar problems in the future, new strainers have been ordered for both fire pumps, and will be installed at a convenient time.
