Mode 1 - Rx Power 20% - Unit Load 110 MWe DESCRIPTION OF OCCURRENCE:

i At 0440 hours0.00509 days
0.122 hours
7.275132e-4 weeks
1.6742e-4 months
, on June 3, 1981, a high level in No. 22 Steam Generator caused a Turbine Trip and a Reactor Trip.

The steam dump opened causing a high steamline flow, coincident with a low Tavg.

This initiated a safety injection, which caused the Boron Injection Tank concentration to decrease to less than 20,000 ppm of boron.

Action Statement 3.5.4.1 was entered.

This constituted operation in a degraded mode IAW Technical Specifi-cation 6.9.1.9.b.

DESIGNATION OF APPARENT CAUSE OF OCCURRENCE:

The safety injection diluted the Boron Injection Tank to less than 20,000 ppm of boron.

ANALYSIS OF OCCURRENCE:

Technical Specification 3.5.4.1 requires that with the Boron Injection Tank inoperable, restore the tank to operable status within one hour or be in hot standby and borated to shutdown margin equivalent to 1% Ak/k at 200 t within the next 6 hours6.944444e-5 days
0.00167 hours
9.920635e-6 weeks
2.283e-6 months
; restore the tank to operable status within the next 7 days or be in hot shutdown within the next 12 hours1.388889e-4 days
0.00333 hours
1.984127e-5 weeks
4.566e-6 months
.

CORRECTIVE ACTION

The Boron Injection Tank was borated to between 20,000 and 22,500 ppm boron from the Boric Acid Storage Tank.

Action Statement 3.5.4.1 was terminated at 2229 hours0.0258 days
0.619 hours
0.00369 weeks
8.481345e-4 months
, on June 3, 1981.
