DESCRIPTION 0.F THE EVENT:

With unit 1 at 100% steady state operation, MOV-CS-102A, the outlet valve from the chemical addition tank (CAT), would not cycle electrically.

This event is contrary to Technical Specification 3,4.A.1 and reportable per T.S.6.6.2.b.(2).

P_ROBABLE C0NSEQUENCES:

The containment spray system in each reactor unit is composed of two separate parallel spray subsystems, each of 100% capacity.

The CAT provides sodium hydroxide to the containment spray system to facilitate iodine removal from the containment atmosphere following a DBA, thus reducing the concentration of airborne fission products available for leakage.

Subsequent to the discovery of the inoperability of MOV-CS-102A, a periodic test established the operability of the redundant CAT outlet valve; therefore, the health and safety of the public were not affected.

CAUSE

The valve motor operator overload protection thermaled open when attempting to cycle the valve. This is believed to have been the result of the valve being overly hand tightened during a drainage sampling program initiated approximately 24 hours2.777778e-4 days <br />0.00667 hours <br />3.968254e-5 weeks <br />9.132e-6 months <br /> previously.

IMMEDIATE _ CORRECTIVE ACTIg:

MOV-CS-102A was manually opened and the motor operator overload protection reset. The valve was found to cycle satisfactorily without resetting the torque switch. The valve was tested via periodic test 19.1 and found to cycle within the specified time.

SHCEDULED CORRECTIVE ACTION:

No scheduled corrective action is required.

ACTION _TAKEN TO PREVEhT R.ECURRENCES:

The standing order to cycle the valve using the motor operator after manual i

operation has been re emphasized.
