April 12, 1981 Identification of Occurrence:

Main Steam Isolation Valve MSV-414 was inoperable contrary to Technical Specifications 3.7.1.5 and 3.6.3.1.

Conditions Prior to Occurrence:

Mcde 3 hot standby (0%).

Description of Occurrence:

At 2230 following a Reactor trip which occurred during a Reactor trip recovery, Main Steam Isolation Valve MSV-414 was discovered to be inoperable. The affected penetration was isolated and main-tenance was initiated.

Designation of Apparent Cause:

The cause of this event is still undetermined at this time. An investigation is in progress; a supplemental report will be is-sued when the cause is determined.

Analysis of Occurrence:

There was no effect upon the health or safety of the general public.

Corrective Action

Modifications were performed to improve valve operation and re-liability. MSV-414 was modified per MAR 79-9-74.

The remaining MSIV's were modified per MAR 81-4-75.

The functional test re-sults were satisfactory and operability was restored at 0300 April 19, 1981.
