l Based on the NRC inspection October 4 - November 3,1980, certain of your activities were apparently not c nducted in full compliance with NRC requirements as indicated below. These iter; have been categorized as described in correspondence to you dated December 31, 1974.

Technical Specification 3.3.2.1 requires that the Engineered Safety Feature Actuation System (ESFAS) instrumentation channels shown in Table 3.3-3 shall be OPERABLE and when inoperable take the action shown in Table 3.3-3.

Table 3.3-3 requires, that with less than the Minimum Channels OPERABLE, operation may continue provided the containment purge and exhaust valves are maintained closed.

Contrary to the above, during the period from about 4:00 a.m. on October 26, 1980, to 11:23 p.m. on October 28, 1980, the Reactor Building Purge Exhaust Gaseous Channel was inoperable and the containment purge and exhaust valves were not closed.

Technical Specification 3.6.3.1 requires that the containment isolation l

valves specified in Table 3.6-1 shall be OPERABLE with isolation times as shown in Table 3.6-1.

Table 3.6-1 specifies the action to be taken when one or more of the valves are inoperable.

10 CFR,73.55(d)(6) requires that individuals not authorized by the licensee to enter protected areas without escort shall be escorted by a watchman, or other individual designated by the licensee, while in a protected area and shall be badged to indicate that an escort is required. The Security Plan requires that all personnel not issued one of the Nuclear Plant badges shall be handled as visitors and escorted at all times while within the Protected Area.
