April 17, 1981 Id'ntification of Occurrence:

The control switch for Emergency Feedwater Pump EFP-2 was in the "Off" position rendering the pump inoperable. This event is contrary to Technical Specification 3.7.1.2.

Conditions Prior to Occurrence:

Mode 3 hot shutdown (0%).

Description of Occurrence:

At 0945 while preparing for a Reactor startup it was discovered that Emergency Feedwater Pump EFP-2 control switch had been left in the "Off" position after completion of SP-349, Emergency Feed-water System Operability Demonstration, at 0655. The switch was immediately placed in " auto".

Designation of Apparent Caese:

The cause of this event is attributed to procedural deficient.es.

Analysis of Occurrence:

There was no effect upon the health or safety of the general publ.c. Redundancy was provided by Emergency Feedwater Pump EFP-1.

Corrective Action

The procedures have been modified to require sign-off steps to verify the control switch is in " auto".

Failure Data:

This was the first occurrence of this type and this is the seventh event reported under this Specification.
