Several were found near anti-vibration bars (AVB) in the tube U-Bends. This is a new problem for Prairie Island. The mechanism for AVB degradation is not yet understood.
This issue will require resolution orior to allowing the plant to return to power.
Limerick Philadelohia Electric Comoany informed NRR that it will tender its application for operating licenses for the Limerick Generating Station on liarch 19, 1981.

i DIVISION OF SECURITY Week Ending March 13, 1981 Integrated Safeguards Information System (ISIS)
Meeting was held with NRC and DOE personnel to explore the classification level of the Nuclear Materials and Management Safeguards System (NMMSS) at Oak Ridge in Order to clarify the hignest level that would be in ISIS and to determine the DOE Secure Automatic Communications Network Sy, stem capacity to handle ISIS traffic.
NRC Manual Chapter 2101 Part XVI, "Prcorietary Information" Contacted Offices and Divisions in order to facilitate the procurement of stamps for documents tnat contain:
- 1) Proprietary Information, 2) Foreign Information and 3)
Trade Secret or Privileged or Confidential Commercial or Financial Information.
GSA Crime Prevention Seminar Division of Security made arrangements with the GSA Federal Protective Service to have a presentation on office crime and sexual assault offered to Regional Office personnel. A similar presentation took place at NRC Headquarters.
National Telecommunications and Information Administration (NTIA) Training Course NTIA is conducting a training program, on April 2,1981, for Government agency person-nel on the telecommunication of sensitive unclassified information. There will be a discussion of the types of inforration involved, vulnerabilities of communications equipment and recommendations for protection.
NRC will be represented by Richard Dopp, SEC, and John Jones, F05.
Automated Infornation Systems Security Plan (AISSP)
The redrafted AISSP was prepared for circulation to Offices and Divisions with a response date of April 3,1981. The AISSP, which addresses the security of systems processing sensitive unclassified data, covers activities to implement an NRC program for the protection of such systems.

The application will include the risk assessment study which was requested in May 1950.
PRAIRIE ISLA*iD, UNIT. 2 STEAM GEtiERATOR INSPECTIO!i An ongoing steam generator (SG) inspection during the refueling outage has resulted in the following major major items of interest:
1.
A sufficient number of defective tubes were found in one of the two SG's to require inspection of all of the tubes in that SG, per the Technical Specification requirements.
2.
Twelve or more tubes will be plugged in this SG since inspection results showed that these tubes had greater than 50% tube wall thickness reduction.
3.
Eighteen or more additional indications of greater than 40% were found in the vicinity of the anti-vibration bars (AVB) in the tube U-Bends.
Although AVB indications were found on San Onofre 1 some years ago this j
prcblem is a new one for Prairie Island.
6 AVB indications have not shown up on previous inspections of Prairie Island or, to our knowledge, on any other facility SG's. The mechanism for the AVB degradation is not understood at this time.
This issue will require resolution prior to allowing the plant to return to power.
IE is involved. Materials Engineering Branch has been reviewing this since first notified on March 2,1981.

l EEAVER VALLEY On March 5,1951, while making repairs to a cracked weld on a 3/4" instrur.ent I
i line in the RHR system (described in PtiO-I 81-27), RHR flow was rt.iuced by approximately one third for a period of 71 minutes due to air binding of an RHR pump. The Reactor Coolant System was vented and water level reduced to the center line of the hot leg to allow weld repair.
The air binding was caused by water level in the the hot leg being lowered below the RHR suction line because of an error in the temporary level indication.- Flow was not completely lost at any time and the core temperature change was from 110'F to 170*F.

Effluent and Environmental (Radiological) Information 1.
Liquid effluents from the TMI site released to the Susquehanna River after processing, were made within the regulatory limits and in accordance with NRC requirements and City of Lancaster Agreement dated February 27, 1980.
During the period March 6, 1981, through March 12, 1981, the effluents contained no detectable radioactivity at the discharge point although individual effluent sources which originated within Unit 2 contained minute amounts of activity. Calculations indicate that less than one millionth (0.000001) of a curie of cesium-137 (CS-137) was discharged This represents less than 0.00001% of the permissable total liquid activity as specified in Technical Specitic3tions for operational coninercial reactors.

Licensee Radioactive Material and Radwaste Shipments. The following shipments were maae:
On Monday, March 9, 1981, a 40 ml Unit 2 reactor coolant sample was sent to Babcock and Wilcox (B&W), Lynchburg, Virginia.
On Wednesday, March 11, 1981, a shielded drum containing assorted materials; e.g., nylon rope, signs, telephone, etc.,
from the Unit 2 Reactor Building was sent to EG&G, Idaho Falls, Idaho.

3 Major Events 1.
Reactor Building Entry. Tne seventh reactor building entry is curren:ly scheduled for March 17, 1981, and March 19, 1981. The major item scheduled for the entry is obtaining a sample of sump water after the water has passed through a zeolite resin column.
2.
Contaminated Building Expansion Joint. The licensee has taken several water-level readings since the installation of the water-level monitor in the expansion joint in the basement of the control building.
The readings indicate that the water level is approximately 9 inches below the basement floor (281 ft. elevation), with a slight daily variation that, to date, does not show any trend.
3.
Contaminated Rodent Feces in the Unit 2 Control and Service Building. The licensee issued a press release on February 27, 1981, concerning the identification of contaminated rodent feces discovered during routine surveys of the Unit 2 Control and Service Building.
To date no rodents have been trapped. Rodent feces, however, have been found which are contaminated with srn1 amounts of cesium-137 (less than one microcurie, one one-millionth af a curie). The licensee called in an exterminator and poison and traps are in place. The contamintion appears localized to small portions of the licensee's protectea area.
On March 10, 1981, the licensee discovered small amounts of contam-ination and rodent feces outside of the missile door (inside the protected area). This area had been contaminated during the accident and accessible areas have since been decontaminated. An 10, 1981, indicated analysis of top soil taken from the area on March small amounts of cesium contamination (approximately 10-4 uCi/gm Cs-137). Analysis of a water sample from the area, after a recent rain, showed Cs-137 in trace amounts of 2.1 E-7 microcurie per milliliter. 8 During the Advisory Panel for Decontamination of Three Mile Island, Unit 2 etering of February 11, the disposal of the existing low-level vaste was discussed.
The discussion centered around the resins con-taining low-level radicactivity, as a result of utilization of the Epicer-2 Sys:cm.
At the above referenced ree:ing, the Panel agreed and in fact authorized ce te cc: respond with yeu cencerning this catter. We feel that the N.R.C. should v.aive the requirement for solidification of the subject vaste when Metropolitan Edison has met all requirements insof ar as avoidance of leakage, safe transportation, shielding and other precautions for shipping the vastes in present containers. We believe the waste should be shipped te a cernercial burial site as soon as possible, provided :here are no dangers to the public during the t:ansportation.
l Metrcpoli:an Edison has submitted a request for such a waiver and we urge ir:rr.edia:e consideration of Met Ed's request.
It is our feeling that Three Mile Island has served as a storage site for an un:cnscionable e..ount of time, and urge the safe removal.of the resins in the present state as soon as safe:y requirements have been met.

As such, I have been directed by the Panel ee=bers to correspend with you cen erning the high-level radioactive vaste at the Three Mile Island site. The high-level vaste is currently con:ained in resins used in the processing of centa=inated water, and stered en site.

The Panel urges you to take imediate action to arrange for the pro:pt and safe re=cval of this radicactive vaste, for interim storage off site pending an ultimate decision en its disposal. We are extremely concerned that Three Mile Island has bece=e a storage site for vaste. We are all aware that Three Mile Island was never intended for such purposes. We believe that SG-08 should be approved for issuance by the IAEA, but the U.S. is proposing specific detailed changes.
SG-08 contains provisions intended to meet the objectives and requirements of the relevant parts of the IAEA Code of Practice on Safety in Nuclear Power Plant Operation (including Commissioning and Decommissioning) related to surveillance of items important to safety in nuclear power plants.
SG-08 addresses the organizational and procedural aspects of surveillance of nuclear power plant items important to safety without providing detailed technical advice on surveillance. The contents of SG-08 are generally comparable to the information found in the applicable NRC Standard Technical Specifications, Appendix B of 10 CFR Part 50, and USNRC Regulatory Guides 1.33, 8.8, and 8.10.
The principal difference between SG-08 and the U.S. documents is in the level of detail.
SG-08 does not conflict with U.S. policy or practice with regard to surveillance of items important to safety in nuclear power plants.
Having been involved in, and satisfied with, the development of SG-08, we are recommending that SG-08 be approved for issuance by the IAEA, but the U.S. is proposing specific detailed changes.

The comment period expires May 4,1981. This Federal Register Notice, along with congressional correspondence and a public announcement, was delivered to the Rules and Procedures Branch on January 12, 1981.
Its publication was delayed by the provisions of the Regulatory Flexibility Act of January 1,1981, which requires that an analysis or a certification by the head of the agency -- stating that promulgation of the proposed rule would not have a significant economic impact on a substantial number of sna11 entities -- be published in the Federal Register at the time of publication of the proposed rulemaking.
State Planning Council on Radioactive Waste Management A representative of NMSS met on March 5 and 6,1981, with the Transportation Task Force forned by the State Planning Council on Radioactive Waste Management.
The Task Force is examining the transportation of radioactive waste in the areas of energency preparedness, promulgation of transportation regulations, enforcement of federal regulations, and liability. Representatives from DOT, DOE, States,' local governments, and public interest groups were on the Task Force. The State Planning Council will report its recommendations on transportation in its final report to be published in July 1981.
US/IAEA Safeguards Agreement Plans for the IAEA ad hoc inspection at Exxon are being finalized.
IAEA representatives will be in Washington on Monday, March 23, for discussions related to their anticipated activities. Both SG and IE representatives will accompany the IAEA to Exxon from March 25 to April 1 to participate in the physical inventory.
It is anticipated that several days during that period will be used by NHSS and appropriate Exxon and IAEA people to discuss and work on the preparation of a draft facility attachment.

EN-Sl-05A Automation Industries, Inc., Sperry Division - An Order Suspending License and to Show Cause why the Suspension Should not be Continued, Pending Further Order was issued to subject licensee on February 17, 1981. This action was based on the licensee's failure to control radiation exposures to preclude further. radiation damage to.the thumbs and index fingers of two employees during cleaning of radiography sources and to preclude a possible overexposure to a third employee.
After evaluating the licensee's response, the staff determined that the licensee's proposed actions were such that further activities involving licensed material would be conducted in compliance with the Corr.ission's requirements.
On March 6,1981, an Order to rescind the suspension of license, terminate the show cause proceedings and temporarily mcdify the license, with an Order to incorporate a requirement for the licensee to implement soecific procedures in its activities and to submit a license amen ment application to incorpora:s those procedures into the existing license was issued to subject licensee.
b.
EN-81-08 Pharmatopes, Inc., - A Notice of Violation and a Notice of Proposed Imposition of Civil Penalties in the amount of $1,500 was issued to subject licensee on March 9, 1981.
This action was based on alleged items of non-compliance relating to transportation of radioactive materials and to the radiation protection program at the licensee's Oak Park, Michigan radio-pharmaceutical facility.

EN-81-09 Consumers Power Company (Palisades Nuclear Power Facility) - A Confirmatory Order was issued to subject licensee on March 9,1981, as a result of "below average" performance over tne past several years.
Over the period September 1979 to September 1980, 41 items of noncompliance were identified at the Palisades plant, two of which resulted in civil penalties.

EN-81-53A Applied Health Physics, Inc., - An Order to Modify License and Order to Show Cause Why Such Modification Should Not be Made Permanent was previously issued to subject licensee bssed on the licensee's failure to transfer all waste byproduct material to an authurized land burial facility as required by an Order issued July 2,1983.
After evaluating the licensee's response, it was determined that the licer.see's proposed actions were such th!

The LOFT T0-4 test in PBF previously scheduled for completion April'3, 1931, har been delayed 5 weeks to May 15, 1931, because of weld.1ng problems encountered in welding the internal themocouples in place. The delivery of the test fuel rods was delayed initially by loss of welded therecouples due to brittleness of the pmtective sheaths. The test date, h'owey~er, remained unchanged until the new welding mandrel for internal thermocouples caused the first themocouple to break off when the second was bei69 welded in place. The problem of mandrel interference has been solved and prepara-tion of the specimens is now underway. It is expected that the completed specimens will be shipped to EG&G by Exxon on March 18, 1931. This will be followed by installation in the refurbished test train and instrument i
check-out, transport, and installation in the PBF loop.

Fire Protection Research Heeting

A meeting was held on March 9 to discuss NRR connents on the fkre protedien research progre. NRR is considering proposing the deletion of att fire replication tests except that for Browns Ferry.
In addition NRR sould like to. revise the BF test plan to include a 1-hour barrier-test. An extensive discussion took place on whether the 1-hour barrier test should be conducted as a separate effects test or be included in the BF f.! ire replication test.. A consensus viewpoint within NRR could not be reached.
This issue is unresolved and will be discussed further within NRR.-

A meeting has been scheduled for March Technical Director of EERI. The objective of the meeting 10to explore possible ways of improving how NRC gathers useful infcer.ation after an earthquake. Current practice is to assemble and dispatch ar@RC Staff Team on an ad hoc basis. However EERI has an ongeing program funded by the National Science Foundation, to dispatch teams of earthquake engineers to the location where significant earthquakes have: occurred.
At this meeting, the possibility of utilizing EERI's earthquake investigations will be examined.
If this can be arranged, it offers potential to reduce duplication of effort.

AEOD provided the ACRS with a detailed technical
- description of the results of its incependent BWR scram system investigations since the BF-3 event.
The presentation, which was made to the Reactor Operations Subcommittee on March 10, 1981, together with representatives from NRR and IE, identified the lessons learned by AEC) and its recommended corrective actions necessary to prevent recurrence.
Nonreactor Assess ent Report AE00 completed an evaluation of an irradiator incident at Becton-Dickinson, Broken Bow, Nebraska, an Agreement State licensee. This incident, which occurred on October 24, 1950, involved a jammed source rack which resulted from damaged product carrier b:xes being used to pass the product to be irradiated around the source. As a res;1t, the shutdown of the irradiator was precluded, and fires resulted from the product Doxes and product being heated to combustion tempera-tures by prolonged exposure to the grnma source. There were no injuries or over-exposures resulting from the incident. The AE0D report will be distributed to NMSS, IE, and S?.
