Summ'ary of Weekly Information Report Week Ending September 12, 1980 Davis-Besse 1 On September 6, while replacing the lubricating oil to the No. 1 Diesel Generator at Davis Besse, maintenance personnel found the sheared-off head of a 5/8" bolt in the oil-sump.

It was found that the Spring Drive Gear Assembly, which is used to transfer power from the main gear to the turbocharger, utilizes four bolts of the 5/8" size, and it was determined that this was the source of the bolt head in the sump. The other three bolts were found to have backed out with resulting severe thread damage. Damage to the main gear was also noticed and work-is under-way to repair all of fie damaged parts.

Region III inspectors are generating an IE Circular on this subject.

(The plant is currently in Mode 5 (cold shutdown) following its first refueling outage which commenced last April.)

Prairie Island Unit 1 On September 4, Northern States Power Company informed the staff that, as a result of ultrasonic testing conducted on the Steam Generator Support Bolts for indica-tions of stress corrosion cracking, 47 of the 48 bolts on Unit 1 were found to have indications of cracking or of breaks in the bolts.

(Unit 1 is in shutdown for its annual refueling and other matters.) The bolts are used in-the upper portion of the structural steel support columns and are required by design to resist tensile service loads only during LOCA loading conditions. The issues of interest are: (a) whether the Unit 1 bolts were pretensioned when installed (to make them more susceptible to stress corrosion cracking) and (b) the nature of the coating on the bolts.

NRR is following the developments as information is made available to the licensee.

Exxon Nuclear - Richland, Washington An updated, completely revised license for the Exxon-Nuclear Company fuel fabri-cation plant was issued on September 10.

A full five-year term renewal license will be considered after completion of the environmental review of the renewal application now being conducted, Development of Acceptance Criteria for the Physical Protection Upgrade Rule i

NMSS has developed acceptance criteria for use in evaluating security plans sub-mitted_in accordance with the Upgrade ble recuirements. The criteria will assist in providing consistent evaluation of upgraded security plans and a means for assuring licensee compliance with every regulatory requirement of the Upgrade

DIVISION OF SECURITY REIK InDJNG. SEPTEMBER 12, 1980 Revision of NRC Manual Appendix 2101, "NRC Security Program," Part IV, presently entitled, " Control of Official Use Only Information," and to be re-entitled,

" Control of_ Official Use Only and Limited Official Use Information," was forwarded to Headquarters Offices and Divisions for comment. This proposed revision has been coordinated with the Department of State, which had no objection to Limited Official Use implementation.

The revision incorocrates the provisions of NRC Bulletin 2101-14, " Protection of Limited Official Use Information," and expands the guidance for both Official Use Only and Limited Official Use information.

While repla'cing the luirtcating oil the No.1 Diesel Generator at Davis-Besse on 9/5/80, maintenance personnel found the sheared-off head of a S/8" bolt in the' oil sump. After checking with the manufacturer (the unit is a GM-EMB Diesel), a check was made of the Spring Drive Gear Assembly (SDGA) which is used to transfer power from the main gear to the turbocharger.

The assembly utilizes four bolts of the 5/8" size, and it was detennined that this was the source of the bolt head in the sump. The other three bolts were found to have backed out with resulting severe thread damage.

Damage to the main gear was also noticed and work is underway to repair all of the damaged parts. The plant is currently in Mode 5 (cold shutdown) following its first refueling outage which commenced last April.

Region III inspectors are generating an IE Circular on this subject. They have detennined that the diesel generator manufacturer had previously issued a bulletin noting the potential for the SDGA bolts to back out since they are provided no locking mechanism.

However, the wording used in this bulletin was not sufficient to draw the immediate attention of utilities utilizing the diesel generator model.

Prairie Island Unit 1 On Septembpr 4,1980,.the staff was informed by the licensee, Northern States Power Company, that, as a result of ultrasonic testing conducted on the Steam Generator Support Bolts for indications of stress t.orrosion cracking, '47 of the 48 bolts on Unit I were found to have indications of cracking or of breaks in the bolts. (Unit 1 is shutdown for its annual refueling and other matters.) The same inspection methods, using the same inspection contractog 'about 14 months ago revealed no similar indi-cations.

The bolts are used in the upper portion of the structural steel support colums and are required by design to resist tensile service loads only during LOCA loading conditions.

The NRR staff discussed the situation with the licensee on the morning of 9/5/80.

Several issues of interest are:

(a) whether the Unit 1 bolts were pretensioned when installed.

(Pretensioning the bolts is thought to make them more susceptible to stress corrosion cracking) and (b) the nature of the coating on the bolt.s.

Unit 2 was shutdown from 100% power, on 9/4/80 to permit inspection of the Unit 2 bolts which are believed to have not been pretensioned.

NRR (0RAB and MEB) is following the developments as information is made available from the licensee.

The Office of Standards Development received the May 1980 Edition of the Calendar of Federal Regulations from the U. S. Regulatory Council.

This document provides an overview of important regulations which agencies are developing, and a summary of the analytical bases for them.

It is designed to encourage greater public participation in the regulatory process, identify areas of multiple regulatory impact, and to assist the regulators in developing cost-effective and consistent regulations.

0FFICE OF NUCLEAR MATERIAL SAFETY AND SAFEGUARDS Items of Interest Week Ending September 12, 1980 Spent Fuel Storage On September 10, 1980, staff members of Fuel Cycle, SD and ELD met with representatives from DOE-Headquarters, D0E-Savannah River Operations and Dupont-Savannah River at their request to discuss NRC regulations, licensing practices and procedures as applied to AFR spent fuel installations. A major focus of interest was the NRC hearing process and the various steps were discussed in detail.

Savannah River is the lead field office for the DOE spent fuel storage program.

The criteria will assist in providing consistent evaluation of upgraded security plans and a means for assuring licensee compliance with every regulatory requirement of the Upgrade Rule relative to the protection of the public health and safety.

Improved Control of In-process HEU Scrap Study An NRC contractor briefed the NRC staff on feasible alternatives for improving control of in-process material at a HEU scrap recovery plant.

Performance factors considered included -timeliness of loss detection, quantity detectable, collusion ~ resistance,.and alarm assessment times.

. Licensing Branch visited Westinghouse-Cheswick to discuss the Fundamental Nuclear Material Control Plan for decommissioning of the Plutonium Fuel Development Labcratory.

Speaking Engagement Pat Vacca presented'a talk on September 5,-1980, on NRC Medical Licensing at the Conference on Health Physics in the Practice of Nuclear Medicine at the 'VA Medical Center, Washington, D.C... Nuclear medicine physicians, radiation safety ~ officers and consultants to VA Medical Centers were present.

They were particularly interested in NRC's medical ALARA program and waste disposal.

Metropolitan Edison Company (Three Mile Island Unit 2) - Civil Penalty Action - On September 11,~1980, the Conmission received a check in the amount of $5,000 from the subject licensee in partial payment of the civil penalty proposed on August 7, 1980. The licensee objected to one of the items of noncompliance and withheld the civil penalty proposed

($4,000) for that item. The licensee's response is under review.

EN-80-39 Magnaflux Corporation, Chicago, IL - On September 10, 1980, a Notice of Violation and a Notice of Propsed Imposition of Civil Penalties in the amount of $2,000 were issued to the subject licensee.

This action is based on an alleged item of noncompliance relating to the noss of a radiographic exposure device during transportation. The device was not blocked and braced as required by Department of Trans-portation regulations.

North Coast Nuclear Plant, Unit 1 10n September li,1980, the applicant, Puerto Rico Electric Power Authority, withdrew its application for a construction permit and moved the presiding ASLB to terminate the licensing proceeding.

Sundesert Nuclear Plant, Units 1 & 2 On September 11, 1980, the applicant, San Diego Gas & Electric Company, withdrew its application for construction permits and moved the presiding ASLB to terminate the licensing proceeding.

Big Rock Point Nuclear Plant On September 12, 1980, the presiding ASLB directed the NRC staff to prepare an environmental impact statement covering the environmental impacts-of an expended. spent fuel pool and the additional term of operation of the facility that such expansion would permit. The ASLB referred to the Appeal Board the rulings underlying that direction. On September 12, 1980, the Appeal Foard accepted the referral and directed the parties to submit briefs and invited the;CEQ.to file a brief amicus curiae expressing its views on the referred 3

The Council adopted positions favo' ring among other things, amendments to the Agreement State program to provide for minimum national standards for dis-posal of low level waste and for the possibility of an agreement to regulate only a low level waste disposal site.

Bill Menczor, State Liaison Officer, Region III, met with the Regional Advisory Committee on Emergency Planning in Kansas _ City, Mo.,

September 8, to discuss the evaluation of the Benton County and Clinton County, Iowa, radiological emergency response plans.

The new payroll system includes on-line data entry and query capabilities and is processed on the mini-computer maintained in the Division of Lccounting.

FY 1982 Budget The budget was formerly transmitted to OMB on September 8, 1980.

FY 1981 Budget The full Senate passed the Energy and Water Development Appropria-tions Bill on September 10, 1980.

The amount recommended for the NRC was (452.5 million'less a reduction to consultant services of

$0.9 million.

Because of the newness of AE00 and the relationship between AE00 and the.

site offices,.such presentations and associated discussions are considered an important aspect towards establishing effective communication channels and assuring mutually supportive efforts.

4 The presentation covered: current AE00 activities to develop LER sequence coding techniques needed for tread and pattern identification; status ~of-AE0D actions on identifying needed changes in NPRDS and LER systems for event reporting; the AEOD procedures and philosophy for reviewing operating events and judging significance; and identification of the fifteen case studies that. are now in progress.

A similar presentation is scheduled to be given to the Region III senior site representatives in late October.
