POWER AUTHORITY OF 'THE STATE OF NEW YORK JAMES A. FITZPATRICK NUCLEAR POWER PLANT-1 DOCKET NO. 50-333 ATTACHMENT TO LER 80-084/03L-0 Page 1 of I During Surveillance. Testing, Reactor Water Level Switch 02-3-LIS-101B or 02-3-LIS-101D was found set less conservative than allowed by l

Technical Specifications.

The variation from the allowed set point and other Information with respect to the instrument problem is shown below:

DATE TIME REMARKS 11/ 3/80 1100 02-3-LIS-101D found set at 10.37 inches.

The t

required set point is equal to or greater than 12.5 inches.

The switch wrs immediately recalibrated.

11/ 3/80 1430 02-3-LIS-101B found set at 11.5 inches.

The required set point is equal te c greater than 12.5 inches.

In addition, the high level trip 2

from the same switch was found set at 58.6 inches when required to be set at equal to or less than 58.0 inches.

The switch was immedi-ately recalibrated.

Other switches performing the same function had been tested earlier on the same day.

In addition, surveillance was increased.

11/13/80 N/A Surveillance satisfactory.

I 11/19/80 N/A Surveillance satisfactory.

11/25/80 1130 02-3-LIS-101B high level trip' set point found set at 58.33 inches wnen required to be set at l

= equal to or less than 58 inches.

Switch was immediately adjusted to within limits and cause of the failure was determined to be the result of air in the sensing lines.

12/ I/80 N/A Surveillance satisfactory.

investigation following the failed surveillance on 11/25/80 revealed the presence of.some air in the Instrument sensing lines.

The air was removed by backflushing with water and the instruments returned to

~

i service following calibration.

Review of the procedure indicates t'at air should not be introduced as part of surveillance testing therefore the source of air is believed to be the result of personnel error during an earlier surveillance.

To preclude recurrence, personnel involved.in the test will review this Licensee Event Report.
