Narrative

Report for LER 50-321/1980-100 i

During normal power operation the _ site was notified by the AE

- ( B e c h t e l)d of a-seismic support that was.never installed.

This was discovered during a review of the "as-built" piping and supports per the requirements of I E B_

79-14.

This _ missing support and excessive clearances on an adjacent support were j

analyr.ed to' ' determine the consequences when subjected to design basis earthquake loads.

This analysis revealed that

the piping str, esses would. exceed code minimum yield values by 10% when pressure, we

)1 t, and DBE loads are combined.

Upon notification of this fact, the AE issued a fix requiring installation of the missing support and modification of the ad j a.c e n t. support.

This was immediately implemented in the

field upon receipt of the-fix.

of the support's not being installed was l

investigated revealing that the subject support was added 1

subsequent to the initial stress analysis printout.

addition was apparently never properly communicated to the

designer or the field.

l Upon receipt of t h'c design from the AE the propos.ed fix was initiated.immediately and completed satisfactorily on 8-21-80.
