One independent ECCS subsystem inoperable contrary to Technical Specification 3.5.2.

Conditions Prior to Occurrence:

Mode 1 power operation (100%)

1 Description of Occurrence:

At 0525 during the performance of ECCS Pump Operability Demonstration Surveillance SP-340, it was discovered that "A" decay heat pump discharge throttle valve would not control flow in automatic.

DRV-110 control was shifted and decay heat system flow was maintained in manual.

Maintenance actions were initiated.

Designation of Apparent Cause:

The cause of this event is attributed to loose wires found on the auto-manual selector switch located in engineered safeguards cabinet-1. An investigation of the event revealed that the wires were probably inadvertently pulled loose during the installation of the diverse isolation modification for NUREG 0578.

Analysis of Occurrence:

No affect upon the plant or general public.

"B" decay heat loop was operable.

Corrective Action

The wires were replaced, tightened and DHV-110 was returned to service at 0635. The workers involved were re-cautioned about the necessity for exercising care while working in the ECCS cabinets.

Failure Data:

This is the fourth occurrence reported for DHV-110. Reference LER 77-150, 78-53, and 79-33.
