APPENDIX A NOTICE OF VIOLATION Philadelphia Electric Company Docket No. 50-277 Based on the results of an NRC inspection conducted on February 1-29, 1980, it appears that one of your activities was not conducted in full compliance with conditions of your NRC License No. DPR-44 as indicated below. This item is an infraction.

Technical Specification 3.2.D.1 states in part, "...both off-gas system radiation monitors shall be operable during reactor power operation...
Upon the loss of both off-gas system radiation monitors, either temporary monitors shall be used...or initiate an orderly shutdown and have the main steam isolation valves closed within eight hours."
Contrary to the above, on January 19, 1980, the Unit 2 reactor was started up and operated at pcwer with both radiation monitors inoperable due to a sample line valving error for a period in excess of 10 hours1.157407e-4 days <br />0.00278 hours <br />1.653439e-5 weeks <br />3.805e-6 months <br /> and no orderly shutdown was initiated.
