During Mode 5 operations at 1633 with shutdown cooling flow secured, it was discovered that charging oumos were running. Charging pumos had not been verified de-energized and the charging flow paths were not verified closed as required by T.S. 4.10.5.1.

Shutdown cooling flow had been stooped at 1250 on 12-15-80 to perform a local leak rate test on penetration number 41 as provided for in T.S. 3.10.5.

Although the requirements of T.S. 4.10.5.1 were not satisfied until 1633, the CR0 was aware of the requirements to not lower RCS boron concentration due to the peculiar plant conditions.

Thus he ensured that no dilution conditions were conducted while 50C was isolated.

Subsequent RCS boron samples verified that no dilution resulted from charging flow during this event. Therefore, this event had no im-pact uoen the public health or safety. This is a non-repetitive event.

CAUSE DESCRIPTION AND CORRECTIVE ACTIONS (CONT'D)

The cause of this event was failure of licensed personnel to perform the surveillance requirements of T.S. 4.10.5.1 after securina shut-down cooling flow. A contributing cause was that the limiting con-ditions for operation of T.S. 3.10.5 do not specifically reouire that l

charging pumps be de-energized and that the charging flow paths be verified closed. These reouf rements are contained only in the sur-veillance requirements of T.S. 4.10.5.

Additionally, these require-l ments are not consistent with the wording of other technical :ceci-i fications, such as T.S. 3.1.1.3 and T.S. 3.9.8.

This event has been reviewed with Operations personnel who were involved.

Furthennore, l

all licensed personnel will be informed of the details of this event.

l Additionally, a request will be initiated to change technical sceci-t fications 3.10.5 and 4.10.5 to make their wordino consistent with T.S. 3.1.1.3 and T.S. 3.9.8.

Finally, approoriate procedures will be changed to ensure tnat both the LCO and Surveillance Reouirements are met prior to isolating SDC to conduct the local leak rate test of penetration 41.
