EVENT DESCRIPTION

On Augusts29, 1980, while. performing the High Pressure Coolant injection

- (HPCI). System pump;operabili ty test,. procedure QOS 2300-2, it 'was ' di s--

covered that:the HPCI. motor gear' unit'would not coma.off the'high speed stop'and controlLflow'at the: desired setpoint.: - The test was perfr,rmed :

by manually increasing turbine speed with.the motor speed change untl1 the. pump flow; reached the'HPCl' flow controller-(FlC'2-2340-1) setpoint

- of 5600 gpm. At this point, the flow controller,swhen operating in the automatic control' mode,11s-designed to' control the flow by positioning-e the motor: gear: unit.to maintain - the des t red turoine.' speed. However, during the; test, the' turbine speed was increased with.the motor speed-changer past = the flow controller setpoint.until the high speed stop was reached wlthout. any response from the motor l gear unit. Work' Request Q07398 was initiated to' repair the ' system,-and the HPCi outage survell-

lance tests required by.the Technical Specifications were begun.

VI.

PROBABLELCONSEQUENCES OF THE:0CCURRENCE:

The.effect.of'this occurrence upon the' safe operation'of the plant was minimal.- Upon receiving lan. automatic' initiation signal, the HPCI' system

would-have started and run past -the flow controller setpoint to the -high s' peed stop position.- Therefore, if needed in the event of a sma11'line-

~

break,ithe.HPCI' system would'have been available..toisupply. water to the rea'ctor ' vessel, although?In excess of the(desired flow rate of 5600 gpm.

HPCl fwas available for.' operation in the MANUAL mode.

In addition, all of; the other redundant' Emergency Core Cooling -Systems were operable

during this event and would have performed their intended function if needed.

Vll. CAUSE:

The causefof the occurrence ls attributed to' component failure. An in-r ternal component of: flow controller FIC 2-2340-1 failed causing the controller to fall high.' 'The controller is Model No. 50-540017HAKL2,

manufacture'd'by~ General' Electric Company.-

Vill.- CORRECTIVE ACTION:

jThe controller was replaced with a Ilke controller from Unit One. The

HPCI system was" subsequently ~ tested satisfactorily. The original con-troller. will be ' repaired under Work Request Q07398 and, then be returned
