On July 24, 1980, at 2:30 p.m., with Unit Two operating at 625 MWe in the RUN mode, routine surveillance test QlS-48, Pressure Suppression Chamber and Reactor Building Vacuum Breaker Actuation and Functional Test was performed. Pressure switch PS-2-1622B was found to actuate at 20.3 inches of water. Technical Specification 3 7.A.3.a. specifies that the pressure suppression chamber to Reactor Building vacuum breakers shall actuate at less than 0.5 psid (13.8 inches of water). Proper recalibration was not successful; the switch was lef t set at 9.4 Inches of water until replaced

'I th a new swi tch. The pressure switch setpoint drif ted previously on

.uly 24, 1979, and was re-adjusted successfully.

VI. PROBABLE CONSEQUENCES OF THE OCCURRENCE:

The Torus to Reactor Building vacuum breaker system consists of two 100 percent flow capacity systems. Pressure switch PS-2-1622A would have actuated at the required setpoint opening vacuum breaker A0-2-1601-20A '

had the need arisen. Safe operation was not affected by this occurrence.

Vll. CAUSE:

The cause of this occurrence was setpoint drift of pressure switch PS 16228. The switch could not be reset properly due to wide fluctuations in the setpoint. The switches are manufactured by Static-0-Ring Company, model number 12BB-KK215V.

Vill. CORRECTIVE ACTION:

The faulty pressure switch was replaced with a new one by Instrument Maintenance and tested satisfactorily in accordance with 415-48 on July 24, 1980.
