3 August 1980 Identification of Occurrence:

Containment isolation valve CFV-ll failed to close contrary to Technical Specification 3.6.3.1, Table 3.6-1.

Conditions Prior to Occurrence:

Mode 4, Hot Shutdown Description of Occurrence:

At 1119 following routine sampling of "A" Core Flood Tank, it was discovered that "A" CFI sample isolation valve, CFV-11, would not close upon remote actuation.

Core Flood Valve, CFV-ll was then manually closed along with Core Flood Valve, CFV-71 to isolate the penetration and maintain containment integrity.

Designation of Apparent Cause:

The cause of this event is attributed to a failed contact rotor in the limitorque operator of core flood valve, CFV-ll.

Analysis of Occurrence:

No effect upon the plant or general public as a result of this event.

Corrective Action

Contact rotor #2 was replaced and the valve declared operable at 0940 on August 5, 1980 after successful completion of SP-370.

An inspection of Core Flood Valve, CFV-ll, limitorque operator has been scheduled for the next planned outage to check for contact rotor wear.

Failure Data:

This is the second occurrence of this type reported for CFV-ll, (reference LER #77-114) and the 26th event reported under specification 3.6.3.1.
