NOTICE OF VIOLATION Based on the results of an NRC inspection conducted during the periods August 2-5 and August 22-23, 1977, it appears that certain of your activities were not conducted in full compliance with the conditions of your NRC Operating License No. OPR-51 and NRC requirements as indicated below:
Technical Specification, Section 6.8.1 requires that " written procedures shall be established, implemented and maintained."

Contrary to the requirement that procedures be established, an approved procedure was not provided to conduct the required weekly, daily and monthly tests specified in Exhibit I of procedure 1304.95, " Fire System Testing and Training."

Surveillance procedure 1304.95, " Fire System Testing and Training," requires that procedure 1304.88, " Fire Water N
System Testing," be conducted once every eighteen months.
I Contrary to this requirement, the'above test has not been conducted since issuance of the operating license.

Surveillance procedure 1304.95 also requires that procedure 1304.93, "Halon Fire System Inspection," be conducted monthly.
Contrary to this requirement, the inspection had not been conducted for the period of November 11, 1975 to August 23, 1977.

Surveillance procedure 1304.95 also requires that the plant superintendent be notified if the required procedures or tests had not or could not be perfonned. Contrary to this requirement, the plant superintendent was not notified during the period November 11, 1976 to August 6, 1977, of the failure to perform the required tests identified above.
This is an infraction.
