NOTICE OF VIOLATION Based on the results of an NRC inspection conducted May 31 - June 3,1977, it appears that certain of your activities were not conducted in full compliance with NRC requirements as indicated below:

Arkansas Nuclear One, Unit One, Technical Specification 6.5.2.8 requires the Safety Review Committee (SRC) to conduct audits of facility activities.
Such activities are specified in part as, "a.
The conformance of facility operation to all provisions contained within the Technical Specifications and applicable license conditions at least once per year.
"b.
The performance and retraining of all members of tbc O
plant management and operations staff,... at least once a year...."
Technical Specification 6.4.1 requires that an operator retraining program be established for the facility that meets or exceeds the requirements of Appendix A to 10 CFR 55.
Contrary to the above requirements, the SRC failed to conduct an audit of the operator requalification program within the period February 1976' through March 1977

10 CFR 50, Appendix B, Criterion V, states in part, " Activities affecting quality shall be prescribed by documented instructions, procedures, or drawings,... and shall be accomplished in accordance with these instructions, procedures or drawings."

The licensee's approved QA Plan, Section 4.2.1 states, " Vendors shall be selected from the Qualified Vendor List (QVL)." The l
licensee's Quality Control Procedure 1004.05, Revision 3, states in part that the QC Engineer "... reviews the requisition to assure... that the intended supplier is on the approved vendor list."

Contrarytotheabove,PurchaseOrders33442(Unit 2)and28494 (Unit 1) were issued to vendors who were not on the approved x
vendor list.

The licensee's approved QA Plan, Section 7.6.1 states, "Q List material shall be handled and stored as prescribed in the J
procedure for handling, storage, and shipping of Q List Materials
~ (QCP 1004.11)." QCP 1004.11, Revision 2, states in part, "5.1 Q List items, identified as such by Q Tags, are stored in areas appropriate..., the manufacturer's recommendations, and any additional requirements which may be established /Jiyjf the QCE."
Contrary to the above, the manufacturer's recommendations associated with the Unit 1 decay heat removal fan cooler motor 4
which was in storage (P015661) were not being accomplished.
This is an infraction.
