Our letter of January 18, 1973, notified you of a situation where a significant nu=ber of piping spools for AS:E Section III piping systems for the Davis-3 esse Euclear Pcver Station had been fabricated at the Kernersville Plant of ITI-Grinnell containing tack velds perfor=ed by velders not previcusly qualified. \_Y ASPE Section III piping for the Davir-Besse Kuclear Power Station is being- fabricated by ICI-Grinnell Corporation at the Kernersville Plant in Ucrth Carclina. Sechtel Corporation, acting as agent for Toledo

. Edison, is perfor=ing routine shop inspection and surveillance of activities at the Kernersville Flant involving this pipe fabrication.

During routine shop inspection and surveillance activities, it was found that sc=e tack velding during fabrication fit-up vas being per-forced by fitters not qualified for velding in accordance with Secticn IX cf the AS:E Code. An in-depth audit following this discovery revealed that 118 spool pieces cf ASE Secticn III, Class 2, and 240 pieces of Class 3 piping were involved. In additien, two spool pieces of ASE Section III, Class '1, piping were tack welded by a velder qualified only in the 1G position withcut positive verificatien that all tack velding was actually perfor:ed in the 1G position. Of these spoel pieces, 72 ASE Class 2 pieces and 97 Class 3 pieces had been shipped to the job site.

Since all velds in AS:2 Class 1 and Class 2 pipe require radiography, these pieces vere accepted on the basis of saticfactory radiographic examination of each veld invcived. of velds, and, in crder to qualif-y this pipe, Grinnell was. required to radiograph a =initu: cf four velds which included tacks made by each fitter involved. In addition, each fitter was qualified in accordance with the nechanical testing precedures contained in Section IX of the #

The Authorized Code Inspector of the ASME has been aware of thi.s situation and has signed the ASFE partial data reports for these pieces involved, confirming that these pipe spools =eet the requirements of ASME Section III.

Extensive procedures have been instituted by Grinnell at the Kernersville Plant to ensure that no velding is done by unqualified personnel e.nd that all procedures are strictly adhered to. Bechtel Shop Inspectors are main-i taining close inspection and surveillance on shop procedures and shop work and Toledo Edison is conducting independent audits of the Eernersville Plant operations.

A report IIo.1300 has been prepared by ITT-Grinnell concerning the subject situation a=plifying the abote information. One copy of this report is enclosed.
