Dear Mr. O' Rork:

NRC Senior Reactor Operator licenses require diligent attention to and regard for the requirements of station operating procedures and Technical Specifica-tions.

Further, Senior Reactor Operator licensed individuals are expected to control and direct licensed activities in accordance with these requirements.

On March 8,1980, you were advised that irradiated fuel had been handled.

During this activity, Secondary Containment Integrity had not been established.

This condition was in violation of station procedures and Technical Specifica-tions.

Your inaction and failure to recognize this violation, after being notified of this occurrence, demonstrated a lack of adequate knowledge of plant conditions and the requirements of station procedures and Technical Specifications.

In your capacity as Senior Reactor Operator and "on duty" Watch Engineer, you are required to be aware of plant conditions and to ensure operations in accordance with the Technical Specifications and statien procedures.

Upon notification that irradiated fuel had been handled, you

. failed to recognize a violation of the requirements of station procedures and Technical Specifications and failed to initiate' appropriate action.

This demonstrated a level of performance below that required of a Senior Reactor Operator u' der these conditions.

n Accordingly, you are hereby notified that future violations of requirements, by you or personnel under your supervision, or similar conduct such as failing to identify violations and initiate appropriate actions, may result in stringent NRC enforcement actions.

These actions may include license modifica-tions, revocation, or suspension and/or civil penalty.. The NRC will not hesitate to take such action for failure to observe any terms or conditions of licenses, or any rule, regulation or Order of the Commission, or any conduct deterrined to be hazardous to the safe operation of the facility.
