Occurrence Date:

Identification of Occurrence:

Failure of FSV-240 control circuit to give proper alarms upon actuation.

Conditions Prior to occurrence:

Mode 6 refueling.

7.

Description of Occurrence:

At 0930 it was determined that Fire Service Valve 240 (FSV-240) was inoperable.

Investigation into the problem by electricians revealed that the control circuit board was burned out.

An operator was assigned to manually actuate the valve in case of fire in the Control Complex.

Control circuit board was replaced and FSV-240 returned to service on 15 April 1978.

2signation of Apparent Cause:

V Meisture 'in control circuit box caused shorting of circuit board.

Analysis of occurrence:

i There was no danger to the plant or public as an operator was assigned to manually actuate FSV-240 in case of a fire.

10.

Corrective Action

Work Request #291 was initiated to seal FSV-240 control box and desiccant bags have been ordered to be placed in box to prevent noisture buildup on control circuit board.
