Identification of Occurrence: Containment Isolation Valves CAV-l&2 inoperable contrary to l

Tech Spec 3.6.3.1.

Conditions Prior to Occurrence:

Description of Occurrence:

At 2200 upon completion of' pressurizar steam space sampling, Chemical,ddition Valves (C17-L and CAV-2) failed to close. Action Statement (b) of Tech Spec 3.6.3.1 was

.cutered. After approrf marely 20 minutes of attempts to close CAV-L and CAV-2 frois the Control Cancer, the valves closed. and the Action Statemenc was satisfied. The valves were then cyclei with the following isolation times: CAV-L,19 seconds, CA7-Z, 8 seconds upon pressure equaliration. Maintenance Voric Requests 0-6168 and 0-6187 vera initiated at this time. Operability of CAV-L was restored on 21 February L978.

Maintanance on CA7-1 will be accomplished during the impending outage and operabilit7 will be restored.

r asignation of Apparent Cause:

Incorrect torqua switch setting of CA7-L operator. Upon checking CA7-Z, it was deter-mined that the process tubing lines were installed reversed.

Analysis of Occurrence:

The safeer implications. of this occurrence are **=T as the pressurizar sample lines re-enter the Reactor CJolant Systene via. the Makeup tank, composing a closed sampling

.systen.

Corrective Action

No corrective action required on CAV-L as maintenance restored operability.

Correceive action for CAV-1 will be accomplished by Maintenance Work Request I-2406 during the.

impending outage.

11.

Failure Deta:

This is the first time CAV-1 has failed to close and the second occurrence of CAV-1 to close as reported by LER 77-36.
