3/29/80 Identification of Event At 1400 on 3-29-80, the Boron Injection Tank (BIT) was declared inoperable due to no recirculation flow through the BIT.

Conditions Prior to Event

The unit was in Mode 1.

Description of Event

At 1400 on 3-29-80, the BIT was declared inoperable due to no recirculation flow through the BIT. Tech. Spec. 3.5.4.1 requires the BIT to be operable.

Tech. Spec. 3.5.4.1 action statement require =ents are net. Health and safety of the public were not affected.

Designation of Apparent Cause The inadvertent draining of the BIT surge tank and the subsequent addition of demineralized water to the surge tank caused boron crystal formation in the BIT recirculation pump 1A suction line.

Analysis of Event

An investigation of this incident revealed the following sequence of events:

1.

On 3-29-80, a maintenance work request was issued to clear the drain line on the BIT recirculation pump 1A.

Af ter attempting to remove the obstruction throughout the morning, the mechanics secured the valves, verified no flow, and left the area.

2.

Shortly thereaf ter, an Assistant Plant Operator (APO) noted water draining from the EIT recirculation pump 1A drain line. The drain line valve closest to the recirculation line was not fully closed due to the valve handle striking the packing gland bolts that were protruding.

Also, the second drain valve (double valve isolation) was apparently not fully seated due to boron crystals being lodged under the valve seat.

The partially open drain valves were then fully closed. The APO began filling the BIT surge tank af ter noting that the surge tank Icvel indicators were reading low.

3.

Af ter filling the surge tank to 68%, an unsuccessful attempt was made to establish BIT recirculation flow. At 1400 on 3-29-80, the BIT was declared inoperable in accordance with the requirements of Tech. Spec. 3.5.4.1.

Also, in order to comply with Tech. Spec.

3.5.4.1 action state =ent requirements, the plant was placed in Hot Standby by 2045 on 3-29-80.

4.

An investigation revealed a partial blockage of the BIT recirculation pump 1A suction line. The line was cleared at 2300 on 3-29-80.

The system was declared operable at 0242 on 3-30-80 following i

re-establishment of recirculation flow and BIT boron concentration.

5.

The cause of the partial blockage was crystalized boron formation due to the addition of cold demineralized water to the pump suction piping.

Corrective Action

1.

The blockage was removed, recirculation flow established BIT boron brought to the required Tech. Spec. concentration and the system was declared operable at 0242 on 3-30-80.

2.

The incident will be discussed with Maintenance and Operations personnel.
