Drop of new fuel elevator test weight on fuel assembly A-48 and reported in accognce with Technical Specification 6.9.1.8.i.

Conditions Prior to Occurrence:

Mode 6 refueling.

Description of Occurrence:

At 1450 hours0.0168 days <br />0.403 hours <br />0.0024 weeks <br />5.51725e-4 months <br />, while lifting the new fuel elevator test weight

(2050 lbs.) from "Y" up-ender to an empty fuel storage rack with the missile shield crane (FHCR-7) a plant fabricated rigging hook failed, dropping the test weight.

The bottom weighted end of the test weight struck an edge of an empty fuel rack and fell over to its side and the top came to rest against an APSR coupling device v

in fuel assembly A-48.

Initial fuel assembly damage assessment failed to reveal any material damage to the struck fuel assembly.

Subsequent inspections of assembly A-48 have revealed that some deformation has occurred.

It has been decided that this assembly will not be utilized in the reactor in its current state and will be replaced.

The test load was removed and stored in the spent fuel area.

Designation of Apparent Cause:

The cause of this occurrence was due to the lack of conserv'atism in personnel judgment of lifting hook selection.

Analysis of Occurrence:

There was no safety hazard to the plant or general public in that immediate sampling of the spent fuel pit water and airborne activity revealed no change from previous condition.

Subsequent periodic sampling affirmed no change in water or airborne ~ activity.

Corrective Action

A review of procedures controlling load testing and identification of all lifting devices has been completed.

Revisions to these procedures will be initiated to assure proper identification and load capabilities of all such lifting devices.

All use of crane hooks, chokers, slings, and shackles performing safety related work will be procedurally controlled.

Disciplinary action has been recommeded.
