2. Facility: Crystal River Unit #3 .
3. Report Date: 2 August 1978 4 Occurrence Date: 10 July 1978
5. Identification of occurrence:

Failure of cable tunnel sump pumps to start creating a condition contrary to Technical Specification 3.3.1.2.

6. Conditicus Prior to Occurrence:

Mode 6 Refueling.

7. De/cription of Occurrence:

At 0145, the Units 1 & 2 shif t Supervisor notified Cnic 3 that while .

performing the Cable Tunnel Sump Pump Operability Surveillance Proca- t dure (SP-322), two of the north sump pumps failed to start. Entered Action Statement in which all operaticus involving core alterations or positive reactivity changes were to be suspended. Units 1 & 2 Mainten-

% ance restored operability of the sump pumps at 0810 and SP-322 was complaced satisfactorily.

8. Designation of Apparent Cause:

Power to the north sump pumps was fcund to be inadvertently secured at the subetation by a person or persons unknown.

9. Analysis of Occurrence:

There was no safety hazard to the plant or general public as no oper-ations involving core alterations or positive reactivity changes were in progress.

10. Corrective Action:

Power to the sump pumps was restored at 0810. The power switches to the sump pumps will be tagged to remain in the on position. Should the power be required to be off,then the Unit 3 Shift Supervisor is to be notified.

11. Failure Data:

This is the first occurrence of an event of this type. . ,
