- 1. Repor No.: 50-302/78-005/03L-0
- 2. Facility: CrystalRiverdnit #3
- 3. Raport Date: 2 February 1978
- 4. Occurrence Date: 9 January 1978
- 5. Identification of occurrence:
Failure of one cable tunnel pump sump to start creating a condition contrary to Technical Specification 3.8.1.1.a.
- 6. Conditions Prior to Occurrence:
Mode 1 operation.
- 7. Description of Occurrence:
i At 0001, the Units 1 & 2 Shift Supervisor notified Unit 3 that when performing the .
Cable Tunnel Sump Pros Operability Surveillance Procedurs SP-322, one of the two sump pumps failed c: start. The one hour Action Statement of Technical Specification 3.8.1.1. was entered md both emergency diesel generators were verified operable and the Breaker Verificacica, SP-321, was performed. Units 1 & 2 maintenance restored operability of the sump pump at 0640 and SP-322 was complaced satisfactorily.
Designation of Apparent Cause:
The current capacity of the start control bulb switch for the pump installed en 4 January 1978 was inadequate, the switch failed rendering the pump inoperable.
- 9. Analysis of Occurrence:
There was no safety hazard to the plant or general public as both "3A" and "33" emergency diesel generators were available and verified operable.
- 10. Corrective Action: -
No corrective serion necessary as installation of a start control bulb switch of adequate current capacity restored operability of the pump.
l.. Failure Data:
This is the second occurrence of this event as reported by LER 78-002/03L-0.
