A second containment elevation 10' hose station remained operable during both events.

CAUSE DESCRIPTION AND CORRECTIVE ACTIONS:

(CONTINUED)

Although the work group was aware of recently initiated controls requiring issuance of pennits for the use of the fire system, they did not interpret and fully implement the specific limitations of these controls. Specifically, the work group used existing hose station equipment (in violaticn of procedures) instead of providing equipment. To prevent recurrence of these events all potential users will be trained on the use of the fire system for non-fire-fighting purposes.

In addition, permanent signs will be installed at each hose station cautioning and instructing personnel in the use of this equipment.
