On January 8, 1980, while leak *esting the RHR service water vault penetrations, procedu QTS 100-33, penetration P-12 failed the leak test. The test was performed by pressurizing the penetration to 15 psig and checking for a,ir leaks with a soap bubble solution. This test was performed to comply with the surveillance requirements in Technical Specification 4.5.H.I.a. A work request was written to repair the penetration.

P-12 is a new electrical penetration installed as part of Modification M-4-2-75-73 to install RHR Service Water Pump seal leak-off collectors and pumps. The penetration was leak tested satisfactory during the modification test performed on June 28, 1979 VI. PROBABLE CONSEQUENCES OF THE OCCURRENCE:

The erobable consequences of this occurrence were minimal.

The unit was shutdown, and there was no fuel in the reactor; therefore, the RHR service water system was not required to be operable. There were no high water problems in the condensate pit since the penetration was last inspected. Had the 2A RHR service water pump been needed, it would have operated as designed. Safe operation of the reactor was not affected as a result of this occurrence. All other vault penetrations were found to have no visible leakage when tested.

Vll. CAUSE:

The cause of this occurrence is attributed to equipment failure.

The hold-down bolts which compress the rubber seal were loose.

This condition caused air to leak through the rubber seal.

Vill. CORRECTIVE ACTION:

The corrective action was to tighten the hold-down bolts. The penetration was retested; and no leaks were found.
