CAUSE DESCRIPTIO 1 AttD CORRECTIVE ACTIOlls When the HPCI operability demonstration was started on 1-22-80, a leak was discovered on a 1/2 inch line to the HPCI booster pump discharge pressure gauge 1E41-R675.

Personnel declared HPCI inoperable at 0600 hours0.00694 days <br />0.167 hours <br />9.920635e-4 weeks <br />2.283e-4 months <br /> on 1-22-80.

This rendered both HPCI and RCIC inoperable.

Tech. Specs. 3.5 E.3 requires an orderly shutdown be initiated and the Reactor be depressurized to less than 113 psig within 24 hours2.777778e-4 days <br />0.00667 hours <br />3.968254e-5 weeks <br />9.132e-6 months <br /> after declaring both systems (RCIC and HPCI) inoperable.

An orcerly shutdown was initiated immediately.

The HPCI leak was repaircd at 1135 hours0.0131 days <br />0.315 hours <br />0.00188 weeks <br />4.318675e-4 months <br /> on 1-22-80.

The shutdown was terminated and the unit returned to normal power operation.

~ NARRATIVE REPORT On 1-21-80, at 0500 hours0.00579 days <br />0.139 hours <br />8.267196e-4 weeks <br />1.9025e-4 months <br /> the RCIC System was tagged out for maintenance to repair several small packing leaks on RCIC valves.

Tech. Specs. 4.5.E.2 requires that when the RCIC System is made or found to be inoperable, the HPCI System shall be demonstrated to be operable immediately and daily there-after until the RCIC System is returned to operable status. When RCIC was tagged out on 1-21-80, the surveillance for HPCI was performed successfully and operability was demonstrated.

The RCIC System remained tagged out until 1740 hours0.0201 days <br />0.483 hours <br />0.00288 weeks <br />6.6207e-4 months <br /> on 1-22-80.

Therefore, HPCI surveillance was required to be per-formed again on 1-22-00 due to RCIC still being inoperable.

While performing the HPCI surveillance, a leak was discovered on a 1/2 inch line to the HPCI booster pump discharge pressure gauge lE41-R675.

The line was cracked and

' spraying water badly.

Personnel declared the HPCI System inoperable at 0600 hours0.00694 days <br />0.167 hours <br />9.920635e-4 weeks <br />2.283e-4 months <br /> on 1-22-80.

Thus, HPCI and RCIC were inoperable simultaneously. Tech.

Specs. 3.5.E.3 requires an orderly shutdown be initiated and the Reactor be depressurized to less than 113 psig within 24 hours2.777778e-4 days <br />0.00667 hours <br />3.968254e-5 weeks <br />9.132e-6 months <br /> from the tin.e that both (RCIC and HPCI) Systems are made or found to be inoperable.

Personnel im-mediately initiated an orderly shut 'own when declaring HPCI inoperative.

However, the HPCI leak was repaired oy Maintenance and the HPCI surveillance was performed successfully.

HPCI was declared operable at 1135 hours0.0131 days <br />0.315 hours <br />0.00188 weeks <br />4.318675e-4 months <br /> on 1-22-80.

The Reactor shutdown wa; terminated and 'the unit returned to normal power operation. The RCIC System was returned to operable status at 1740 on 1-22-80.
