Farley Unit 1 Report Date: 3/14/79 Event Date: 2/16/79 Identification of Event Containment main personnel and emergency air lock outer doors failed to meet the acceptance criteria of Tech. Spec. 4.6.1.3.a.

Conditions Prior to Event The unit was in mode 1 at 89% power.

Description of Event FNP-1-STP-118.0, " Containment Airlock Leakage Test", was completed satisfactorily for both air locks on 2/16/79. In order to declare both air locks operable, FNP-1-STP-15.0, " Containment Air Lock Door Seal Operability Test", was required to be performed satisfactorily as well. FNP-1-STP-15.0 was performed on the main air lock at 0600, 1750 and 1840 with unsatisfactory results. (The initial performance of this STP was marked satisfactory by the Assistant Plant Operator due to his misinterpretation of the acceptance criteria.)

FNP-1-STP-15.0 was performed on the emergency air lock at 1535 with unsatisfactory results. During the Shift Foreman review of FNP-1-STP-15.0 at 1600 on 2/16/79, it was determined that both air lock outer doors had failed to meet the acceptance criteria of Tech. Spec. 4.6.1.3.a (pressure leakage of 2 lbs. and .5 lbs. ,

respectively vs. no detectable leakage permitted). After cleaning tnd lubricating of both door seals, and a successful performance of FNP-1-STP-15.0 on both air locks, the doors were declared operable at 2015 and 2100 on 2/16/79.

Designation of Apparent Cause Leakage was caused by probable dirty door seals and/or temporary set in the seals caused by the immediate earlier performance of FNP-1-STP-118.0. Performance of FNP-1-STP-118.0 requires pressurizing the air lock interior to 48 PSIG, hence the temporary set. The temporary set is not attributable to performance of FNP-1-STP-15.0 in that the air space between the two seals on the outer doors is pressurized.

79-008/03L-0 Analysis of Event Tech. Spec. 3.6.1.3 action statement requirements were met. The doors were returned to an operable status within the time allowed by Tech. Spec. 3.6.1.3. The health and safety of the public were not affected.

Effect on Plant This occurrence had no significant effect on plant operation.

Corrective Action

a. The seals were cleaned and lubricated.
b. The Assistant Plant Operator was instructed regarding the proper acceptance criteria in FNP-1-STP-15.0.
c. FN STP-15.0 will be revised to clarify the acceptance criteria.

Failure Data A similar occurrence was reported in LER 78-081/03L-0.
