Missed Technical Specification Surveillance Requirement CONDITIONS PRIOR TO OCCURRENCE:

Operational Mode 1 Reactor Power 100%

DESCRIPTION OF OCCURRENCE:

During a routine inspection of the Control Room activities on 2 79, the Resident NRC Inspector found that the four hour rod position read-ings required by Technical Specification Surveillance 4.1.3.1.1 were not logged at 1200 hours0.0139 days <br />0.333 hours <br />0.00198 weeks <br />4.566e-4 months <br /> on 2-21-79. A review of previous console reading logs by the Operations staff showed that the 1200 hour0.0139 days <br />0.333 hours <br />0.00198 weeks <br />4.566e-4 months <br /> reading had also been missed on 2-3-79, Corrective action was taken to emphasize the required surveillance; but on 3-3-79, the 1200 hour0.0139 days <br />0.333 hours <br />0.00198 weeks <br />4.566e-4 months <br /> reading was again not logged.

DESIGNATION OF APPARENT CAUSE OF OCCURRENCE:

The cause of this occurrence is personnel error. The control console log has a numbered notation which states that the rod position readings must be logged every four hours if the Technical Specification surveil-lance is applicable. The operators were aware that the readings were required but were not logged due to other plant evolutions occurring at this time. This is the only log entry required for 1200 hours0.0139 days <br />0.333 hours <br />0.00198 weeks <br />4.566e-4 months <br />.

ANALYSIS OF OCCURRENCE:

Technical Specification Surveillance 4.1.3.1.1 requires that the position of each full length rod shall be determined to be within the group demand limit by verifying the individual rod position at least once per 12 hours1.388889e-4 days <br />0.00333 hours <br />1.984127e-5 weeks <br />4.566e-6 months <br />, except during time intervals when the Rod Position Deviation Monitor is inoperable, then verifying the group position at least once per four hours. The indication was not logged for one four hour period for each of the days cited in this report. All readings logged for^these days verified rod position to be within the group demand limit.
