22 September 1979 Identification of Occurrence:

"A" decay heat cooler outlet temperature indicator inoperable contrary to Technical Specification 3.3.3.5.

Conditions Prior to Occurrence:

Mode 1 power operation (100%).

Description of Occurrence:

At 0230 it was reported that the remote shutdown indicator for "A" decay heat cooler outlet temperature, DH-39-TI had failed.

Further investiga-tion revealed that RID DH-2-TE had failed. Maintenance restored opera-bility and the indicator was returned to service at 1330 on 28 September 1979.

Designation of Apparent Cause:

I e cause of this event is attributed to a failed RTD, DH-2-TE.

Analysis of Occurrence:

No effect upon the plant or general public.

Corrective Action

The RTD was replaced with a spare and returned to service. No further corrective action deemed necessary.

Failure Data:

This is the second occurrence of this type reported.
