Alarm Point F-018 was in alarm with no detectors in alana state rendering it inoperable The unit was in Mode 1, with Power (MWT) = 2725, Conditions Prior to Occurrence:

and Load (Gross MWE) = 912.

While reviewing ST 5016.03, "Non-supervised Fire Alarm Description of Occurrence: the designated reviewer noted Circuit Checks", on January 5, 1979 at 1500 hours0.0174 days
0.417 hours
0.00248 weeks
5.7075e-4 months
, that a deficiency had been written on December Investigation 28, 1978 stating that Contact on January Dataat 5, 1979 Logger Point "F-018" was in continuous alarm.F-018 was the Containment the local Fire Alarm and th 1500 hours0.0174 days
0.417 hours
0.00248 weeks
5.7075e-4 months
showed that Panel C4709, which feeds C4705 through C4706, was working and no alarms '

Containment This indicated that F-018 was receiving a false alarm signal and should were present. in the Action Statement of Technical have been declared inoperable, placing 28, the unitwhich 1978, requires a fire watch patrol.

Specification 3.3.3.8.a on Decemberidentified as a Technical Specification detector This was not done as it was not then.

A deficiency was written on December 28, 1978, but no work request was submitted until January 4,1979, when the designated reviewer noticed, during a cursory It was not until 1500 hours0.0174 days
0.417 hours
0.00248 weeks
5.7075e-4 months
on review , that no work request had been written. was being fonnally reviewed that January 5,1979, when Surveillance Test ST 5016.03 it was recognized that a Technical Specification detector was involved.

Troubleshooting revealed two Indicating lights inoperable on Panel C4705 and that Resetting Panel C4705 at 1547 hours0.0179 days
0.43 hours
0.00256 weeks
5.886335e-4 months
on January 5,1979 Panel C4705 was not reset.

cleared F-018 and removed the unit from the Action Statement. This was within the allowable time requirement of one hour so no fire watch patrol was established.

The f ailure of the indicating lights Designatiop of Apparent Cause of Occurrence _:

on Panel C4705 prevented plant personnel f rom knowing that Panel C4705 needed to be reset and had not been reset after alarm on Panel C4706 had cleared and been reset.

Failure to identify F-018 as a Technical Specification alarm allowed a delay in initiating corrective action.
