APPENDIX A NOTICE OF VIOLATION Power Authority of the State of New York Docket No. 50-333 Based on the results of an NRC inspecticn conducted on September 10-20,1979, it appears that certain of your activities were not conducted in full compliance with conditions of your NRC Facility License No. DPR-59, as indicated below.

These items are infractions.

A.

10 CFR Part 50, Appendix B, Criterion V, states:

" Activities affecting quality shall be prescribed by documented instructions, procedures, or drawings of a type appropriate to the circumstances and shall be accomplished in accordance with these instructions, procedures, or drawings.

Instructions, procedures, or drawings shall include appropriate quantitative or qualitative acceptance criteria for determining that important activities have been satisfactorily accomplished."

Quality Assurance Program, Section 5, entitled, Instructions, Procedures, and Drawings, states in Paragraph 3.0:

"The following are examples of major quality affecting activities that Authority personnel shall perform relating to this section of the manual," and states in Paragraph 3.1.5 as an example referred to in Paragraph 3.0:

" Identify, in appropriate procedures, individuals or groups responsible for performing specific tasks and provide sufficient detail to ensure that quality related activities are satisfactorily performed."

Contrary to the above, on September 12, 1979, procedures and their associated valve lineups did not provide sufficient detail to ensure that quality related activities are satisfactorily performed in that the instrument root valves (the first valve in the piping supplying instrument sensing signals off of the process system), were not included on the valve lineups and, therefore, not checked for' proper positioning prior to facility operation.

B.

Technical Specification 6.8(A) states, in part:

" Written procedures and administrative policies shall be established, implemented, and maintained..."

Pr.ocedure F-0P-14, entitled, Core Spray System, requires, in Table I, Valve Lineup, that valve number CSP-716B, Torus Suction Line Drain, be locked closed.

Contrary to the above, on September 12, 1979, valve CSP-716B was found to be closed but not locked closed as required by Table I of F-0P-14.
