POWER AUTHORITY OF THE STATE OF NEW YORK JAMES A. FITZPATRICK NUCLEAR POWER PLANT DOCKET NO. 50-333 ATTACHMENT TO LER 80-005/03L-0 Page 1 of 1 During a management review of reactor water chemistry data, it was discovered that during a reactor startup which was conducted on December 30, 1979, some of the reactor water samples required by Technical Specifications Paragraph 4.6.C.1.c were not conducted. The referenced paragraph requires samples every four (4) hours.

From 0400 hours0.00463 days
0.111 hours
6.613757e-4 weeks
1.522e-4 months
until 1430 hours0.0166 days
0.397 hours
0.00236 weeks
5.44115e-4 months
(10.5 hours5.787037e-5 days
0.00139 hours
8.267196e-6 weeks
1.9025e-6 months
) no samples were taken.

During the reactor startup of concern, which was commenced at 0211 hours0.00244 days
0.0586 hours
3.488757e-4 weeks
8.02855e-5 months
on December 30, 1979, reactor water samples were obtained as listed below:

Date Time Remarks 12/29/79 2215 Prior to startup 12/30/79 0211 Startup initiated 12/30/79 0400 12/30/79 1430 12/30/79 1830 12/30/79 1950 Mode switch in run Analysis of the above listed samples showed a reactor water conductivity of less than 1 micromho/cm, a chloride ion concentration of less than 0.1 ppm and a gross gamma ;ount which indicated that the total iodine concentration was less than 0.007 microcuries/ml.

The event did not represent a significant hazard to the public health and safety.

To preclude recurrence, the Radiation Protection Supervisor has discussed this event with the Radiological and Environmental Services Technicians and stressed the importance of obtaining each of the samples as required by Technicial Specifications.
