SUPPLEMENTARY INFORMATION REPORT NO: 50-302/79-103/03L-0 FACILITY: Crystal River Unit 3 REPORT DATE: 16 November 1979 OCCURRENCE DATE: 24 October 1979 IDENTIFICATION OF OCCURRENCE: Degradation of the makeup system (makeup pump 1B discharge line vent valve MUV-291) reportable under Technical Spec-ification 6.9.1.9.d. CONDITIONS PRIOR TO OCCURRENCE: Mode 1 power operation (100%). LESCRIPTION OF OCCURRENCE: At 1735 during routine power operation, a leak was discovered on makeup pump 1B discharge line vent valve MUV-291. Further investigation revealed that the leak originated from a failure in the weld area near the root of the weld. The crack was repaired through modification 79-10-84. Maheup pump 1B was returned to service at 1340 on 30 October 1979 following sat-isfactory completion of SP-340. DESIGNATION OF APPARENT CAUSE: The cause of this event is attributed to cyclic vibration fatigue. ANALYSIS OF OCCURRENCE: No effect upon the plant or general public as redundant makeup pumps MUP-1A and MUP-1C were operable. CORRECTIVE ACTION: Request for ent.ineering information 79-7-7 has been submitted to evaluate this event. Corrective action will be taken upon completion of this evaluation and submitted as a supplement to this report. FAILURE DATA: This is the fourth occurrence of this type reported for MUP-1B. Reference LER 79-63, 82 and 93.
